Parent/Guardian Permission - SIGN for Registration to be Processed – Good Through June 2015
In consideration of the acceptance of my son or daughter in the programs I have selected, I hereby agree:
-
to allow Mason Middle School to use my child’s picture and/or name in publications, displays, presentations, newspaper articles,
brochures and other related publicity regarding Community Education activities.
to allow my son or daughter to participate in supervised off-site walks or outings as a part of the activity for which he/she is
registered, including scheduled bus trips to locations both within and outside of Mason Public Schools.
to allow my son or daughter to participate in supervised use of the computer and the internet as a part of their chosen activity.
I recognize that participation in programs outside of the school day, specifically including athletic or physical education activities,
requires an increased level of exercise and physical activity. I, as the parent or guardian, hereby certify that the child specified herein
is in satisfactory physical condition, capable of moderate exercise, running, jumping, and other physical activity, which may be
included in the activity for which he/she is registered. If I am unsure of the content of the program that I have selected, I will inquire
for further information from Mason Middle School prior to participation. If I am unsure of my child’s fitness level or ability to
participate, I will seek the advice of a physician or qualified medical professional prior to allowing him/her to participate. I understand
that any of the SOAR activities may include some physical activity, not just those classified as ‘sports,’ as a part of the choices
available for students participating. I understand that the District cannot be held responsible for unforeseen circumstances or events
that may occur during the activity time. I agree that the Mason Middle School staff will use their best judgment and training in
providing a safe experience for all of the participating students. I understand that if emergency medical treatment is determined to be
necessary by the staff and I am unable to be contacted, the staff will proceed with securing such emergency care, including
hospitalization, as may be needed under the circumstances.

volunteers or other methods of transportation are not school buses, and as such, are not in compliance with
all current regulations for school buses. I feel that the additional risk of transporting my student in a car

District’s choosing, the School District for any and all claims arising out of me or my son/daughter’s
providing transportation herein.
I, as parent or legal guardian, acknowledge my responsibility for legal liability imposed on me as a result
of personal actions taken by me and/or my student during this trip/event/activity. I also release the School

student injuries. I understand that, as the parent/guardian of a field trip participant, I am responsible for all
emergency or medical fees and/or costs incurred as a result of emergency circumstances.
I hereby give permission for my child, __________________________, to travel off school grounds by the
