Chicago All Inclusive Health is a community-based and individual-centered organization that strives to
positively and pro-actively enhance the health and wellness of individuals in the community, in addition
to health practitioners. We provide the environment to foster the growth of your holistic health practice
without the legal and financial burdens of establishing an independent practice. !

Chicago All Inclusive Health acts as an umbrella - or the location - from which independent practitioners
do business. Much like a shopping mall has a name, but within it exists multiple independent retailers
who do business under a different name. You will not forfeit your unique business name, you will not
lose your web address, essentially, you will not merge with CAIH upon joining.!

Chicago Bodywork - the therapeutic bodywork practice from which the concept of Chicago All Inclusive
Health, Inc. developed - has spent more than 3 years building a large client base. Chicago Bodywork’s
client base has interest in other health modalities, such as acupuncture, personal training, nutrition
counseling, and mental health counseling. While referrals cannot be guaranteed, some of the benefits
of aligning your practice with others, are the possibility of a greater client base and additional
networking opportunities.!

Select one or more weekly blocks from the schedule below. Each block selected is yours every week at
the same time for a minimum 3 month contract. By selecting more than 2 blocks, you will receive a
percentage off additional blocks as follows:!

We have three treatment rooms available. Two are furnished with a treatment table, rolling stool, rug,
client seating, storage units, folding desk, and lamp. One is furnished with three chairs, a table, rug,
and lamp and is appropriate for a talk therapy/nutrition counseling room.!

Example of Potential Revenue from One Weekly Block!
!
A weekday block Monday through Thursday afternoon (1:15pm - 5:15pm) can accommodate two 60
minute appointments and one 90 minute appointment with 15 minutes in between sessions. If your
appointment fee is $80/hr and $115/90 minutes, you could potentially earn $1100/month from this block
if fully booked (Four - 90 minute appointments plus eight - 60 minute appointments). Your fee to us for
this block is $110, leaving you with $990, or 90% of your revenue!!

If we refer a client*, our involvement is done after their first session with you, and it is up to you to
maintain the individual as a client. As a referral courtesy to us, we charge a one time fee of $25 for
each client referred. In addition to booking with you directly, membership provides the option for
potential clients to book with you online through Chicago All Inclusive Health’s website, based on the
availability you provide. You are responsible for establishing prices, cancellation policies, sending
appointment reminders, maintaining communication, and taking payment from your client.!
