The producer code randomly selected within the month time frame that submits at least one watercraft policy for ARROWHEAD General Insurance Agency, Inc.’s Marine program will be awarded one $250 Amazon gift card. A total of three $250 Amazon gift cards will be given out during the contest period. Promotion Period. At least one policy submitted in a month timeframe during our Promotion Period, 12:00 AM April 1, 2014 to 11:59 PM June 30, 2014. Limit. There is no limit to the amount of policies submitted during the Promotion Period. Participants and Eligibility. Participants are defined as producers of an active agency contracted with Arrowhead General Insurance Agency, Inc. with an active producer code for the Marine Program. Each active producer code will be treated as a separate Participant. Contest is only open to licensed producers, who are 18 years or older as of April 1, 2014. Employees of Arrowhead and its affiliates, its dealers, its advertising and promotional agencies, its judging organization, manufacturers or distributors of Contest materials and their immediate families in the same household are not eligible. You are not authorized to participate in the Contest if you are not located within the United States. Contest Qualifications. A total of three $250 Amazon gift cards will be awarded to multiple producer codes. Each month one producer code will be drawn who have submitted a minimum of one Marine watercraft policy during the month period. Entries will be given to producer codes based on the following qualifications. A Participant will receive one entry into the drawing if they have submitted one watercraft policy via manual form or email and two entries if they have submitted online at ArrowheadExchange.com during the month in the Promotion Period for the Arrowhead Marine Program. Producer codes may win only once during the promotional period. Prize Details. Three $250 Amazon gift cards will be awarded during the promotional period. The Prizes include all taxes, and government fees. Prize(s) cannot be transferred, substituted or redeemed for cash except at Sponsor’s sole discretion. Sponsor reserves the right to substitute a Prize with a Prize of equal or greater value. Prize is not transferable. In accordance with the IRS Gift Tax, the winner will receive a W9 and will be required to provide their taxpayer information. Drawing and Awarding of Prize. The winners of the Prizes will be chosen from a random drawing from all entries during the month period, on the last day of the month. The announcement will be made on Arrowhead’s official Facebook page and blog on the last day of the month during the promotional period. Odds of winning the Prizes are based upon the overall policies submitted during the promotional period. If potential winner cannot be reached within fourteen (14) calendar days from the first notification attempt, then such person shall be disqualified and an alternate winner will be selected. Winner may be required to sign an affidavit of eligibility and liability/publicity release within 14 days after notification is mailed, or prize will be forfeited and awarded to an alternate winner.
