You are on your way to achieving the goals and direction you want in your life and I am excited to provide you with resources to help you reach your true potential. Below are some policies and procedures that will allow our work to progress in a timely manner and optimize our sessions together. Please complete all necessary paperwork prior to your session (new client paperwork, client report sheet, wellness forms, etc.) *Payment is due at the beginning of each session: o 30 min session is $40.00 o 60 min session is $80.00 o 90 min session is $120.00 Checks (preferred form of payment ) are made out to Performance Coaching 4U If you use a credit/debit card, a 2.8% charge will be added to each transaction. 24 hour cancelation policy is in place. The first cancelation with less than a 24 hour notice is a grace cancelation. The next cancelation without a 24 hour notice will result in a $35.00 cancelation fee. Consecutive canceled appointments will result in a loss of services. If you arrive late to a session, you will be charged for the length of the schedule session. A 25% discount will be applied to your next session if you refer a friend or family member to my consulting services.
