Addendum No. 1
ADDED TO THE PLANS AND SPECIFICATIONS FOR THE ROCK CANYON
ELEMENTARY SCHOOL. Provo City School District (PCSD)
TO ALL CONTRACTORS SUBMITTING BIDS ON THE ABOVE PROJECT: This
Addendum No. 1 is hereby made a part of the Contract Documents pertaining to the
above project and shall be binding upon each Contractor submitting bid. It shall be
the responsibility of each Contractor to notify his subs and suppliers concerning the
contents of this Addendum as they specifically apply to them.
ITEM No. 1 – The project bid date is Thursday June 11, 2015 contractors must
submit their bid prior to 3:00 pm. All bids are due to Provo City School District 280
West 940 North Provo, Utah ATTN: Tina Fluehe Provo City School District
Purchasing Director. This project will be bid at the same time as Sunset View
Elementary School but separately on its bid form. See specifications.
ITEM No. 2 – The following general contractor companies have been pre-qualified
by Provo City School District to bid the Rock Canyon Elementary School and Sunset
View Elementary School. Only these contractors are qualified to bid the above listed
schools.

The contract documents prepared by Kevin Madson and Associates, Inc. (KMA
Architects) will be available to contractors May 28, 2015 at noon for bidding. Plans
and specifications can be obtained from architect’s office:

ITEM No. 3 – The pre-qualified general contractors can bid both Rock Canyon
Elementary School and Sunset View Elementary School for Provo City School
District using each Elementary’s bid form. See specifications. These contractors can
also bid both Elementaries as a combination bid using combination bid form provided
by PCSD.
ITEM No. 4 – The Provo City School District has the right to select one of the prequalified contractors for one school and the right to choose one of the pre-qualified
contractors for both schools. PCSD has the right to reject any of the bids or all bids.
