Prom Information
Ticket Sales: April 20 - April 24
during your lunch period
(STUDENTS ARE NOT PERMITTED TO BRING ANYONE OVER THE AGE OF 20)
No bare feet will be allowed at prom. Please bring socks or flip flops!
Early dismissal will be permitted at 11:35 for students attending the prom only if
the attached permission form is completed.
DO NOT MAKE HAIR/NAIL APPOINTMENTS BEFORE 11:35 AM!
In order to purchase prom tickets, both you and your date must complete the attached permission
slip. They must be signed by a parent or guardian regardless of your age.
You may not purchase tickets without a completed permission slip.
Directions:
1. Complete the attached permission slips: Prom Order Form, Prom Contract, & Early
Dismissal Form (make sure to attach all appropriate ID’s)
2. Please read all rules and regulations carefully. Please note the times of each function.
3. This packet of information and all permission forms can be found on the school website.

Prom Rules
The prom is a school activity, therefore, the same behavior /policies that are observed in school are required,
regardless of the age of the guest. Students should comply with the following rules:
Eligibility
All juniors and seniors at Schalick High School who are not on the restricted/fine list are eligible to attend the prom
with a guest.
Middle School students are not permitted to attend.
ALL guests CANNOT be over the age of 20. (except for a student currently enrolled in Schalick High School).
Guests must be enrolled in a regular, day-high school program---- if your date does not meet this criteria, you may
apply for a WAIVER by completing a prom contract for your date.
EVERYONE (Schalick students and guests) must present their current high school photo ID or driver’s
license in order to gain entrance into the prom.
Guests who are not enrolled in Schalick High School must receive a favorable recommendation from the high school
they currently attend or have most recently attended.
You must attend the prom assembly in its entirety in order to be admitted to the prom. Refunds will not be given for
those students who do not attend the prom assembly.
Dress Code
The prom is a formal event. Your attire and behavior should reflect this.
Violation of dress code will result in denial of entrance into or removal from the prom.
In order to avoid embarrassment on your part, we ask that you use good taste when preparing for the evening.
Sheer and suggestive clothing would be inappropriate in keeping with the formality of this event. If your formal wear
is deemed inappropriate by the staff and/or administration you will be asked to wear a cover up or change.
Changing of attire after the prom begins is NOT permitted.
GUYS:
NO denim
formal footwear required
tuxedo or suit or jacket with tie
jackets may be removed
shirts must be buttoned-down and remain ON and BUTTONED all evening
shoes, flip flops, or socks must be worn at all times
GIRLS:
formal dress or pant outfit/NO denim
the back may be bare only to the waistline
the top of any two-piece dress will be no more than 3 inches from the waistline of the skirt.
no low or revealing tops/dresses
sheer material is not permitted
shoes, flip flops, or socks must be worn at all times
General Information
Prom tickets are NON-transferable
All students attending the prom are required to arrive no later than 6:30 p.m.
Due to security reasons, no one is allowed to leave the building without permission. If you do so, you will not be
permitted to return and your parents/guardians will be notified.
In accordance with state law, any student driving with a provisional driver’s license is not permitted to drive after 11pm
and may not have more than 1 passenger in the car. Therefore, the prom will conclude at 10:30 pm.
ANY STUDENT planning to leave earlier than 10:30 pm is required to have a note from his/her parents stating the
time they may leave and a phone number at which parents can be reached.
If any student is remaining after 10:30pm, their parents/guardians will assume financial responsibility for the
security required to remain and supervise their child.
No refunds will be given after May 8, 2015, NO EXCEPTIONS!!
Dance Protocol--Inappropriate dancing will not be tolerated--NO CLUB DANCING OR GRINDING! Inappropriate
dancing will disqualify those eligible for Prom Court from being honored and may result in a discipline referral.

I have read the information concerning the rules about the prom. I understand that any violation of these
rules will result in disciplinary action as stated in the student handbook.
Any twelfth (12th) grade student found in violation of the drug/alcohol policy as part of the prom activity
will not be permitted to participate in graduation ceremonies or other senior activities. Any student found
in violation of the drug/alcohol policy as part of the prom activity will not be permitted to attend any
future prom.

For any non-Schalick guests a copy of a photo ID is required at time of ticket purchase in order for guest
to be considered. All non-Schalick guests are subject to a background check. You will only be notified if
your date is not eligible to attend, in which case a refund will be issued. The school administration
reserves the right to make the final decision. No refunds will be given after May 8, 2015 for any
reason. Once payment has been made, students that go on the restricted/fine list will not receive a refund
and neither will their guest! Prom tickets are nontransferable.
EVERYONE MUST PRESENT A CURRENT HIGH SCHOOL PHOTO ID OR DRIVER’S
LICENSE IN ORDER TO BE PERMITTED TO ENTER THE PROM! Signatures above verify
that students and parent/guardian have read and agree to the rules outlined in the Prom Packet.
