How to Refer to the Swindon Nutrition Support Community Dietitians
Who are we?
Registered Dietitians (RDs) are the only qualified health professionals that assess,
diagnose and treat diet and nutrition problems at an individual and wider public health
level. Uniquely, Dietitians use the most up to date public health and evidence based
research on food, health and disease, which they translate into practical guidance to
enable people to make appropriate lifestyle and food choices.
When should people be referred to the Nutrition Support Community Dietitians?
As a guide, people who have a ‘MUST’ (Malnutrition Universal Screening Tool) score of 2
(high risk) or more should be started on 2 x 1.5kcal/ml oral nutritional supplements (using
Ensure Plus first-line, as per current community contract). If after one month no progress
has been seen, a referral to the Dietitians should be made, either for an outpatient clinic
appointment or for a home visit as appropriate.
Care homes: Please note that the care homes in Swindon have had ‘MUST’ training and
have been instructed to refer anyone with a ‘MUST’ of 2 or more to the GP to start on oral
nutritional supplements (ONS) as detailed above. ‘Food first’ (nourishing snacks and
nourishing drinks in between meals, and food fortification) should also be encouraged.
Residents with ‘MUST’ score of 1 (medium risk) or 2 (high risk) should be weighed weekly
in the care home.
