How to Complete the Internship USA Application You may type into the form, but must print and sign the form in order for your application to be complete. Some fields within the application, including signature fields and all fields of the Medical Statement (page 8) must be printed and completed by hand. The Medical Statement (page 8) must be completed by a medical professional in English. Once we have received, reviewed and accepted your application, arranged and completed an interview, we will upload your application with the official visa sponsor, WISE Foundation. WISE will then be in contact with your host site to develop and complete the required DS-7002 Internship training plan. When this plan is approved by WISE and your host site, SWAP will email it to you for your signature. Applications must be submitted in full, please review the document checklist at the end of the instructions to ensure that you are submitting everything required. You will be contacted shortly after SWAP receives your application in order to let you know whether or not you have been accepted and to arrange a participant interview (page 9). SWAP is fully committed to assisting you throughout every aspect of the application process and will do everything possible to ensure that your application is successfully lodged. Training dates should be no longer than the maximum of 12 months, and you should apply up front for the full duration of your intended internship. You can enter the US (Date of departure to U.S.) a maximum 14 days before the training start date and stay up to 30 days after the training end date. You must be insured for the entire time of your stay, and your program fees will be determined by your entire stay in the US, not just the amount of time you will be at your internship. Please make sure you check out the instructions on how to calculate your registration fee based on your departure and return dates below. PROGRAM APPLICATION You will be applying for INTERNSHIP, if applying for the PCT Training visa please print out the appropriate application forms and instructions. Applicant Information Complete as appropriate. All information should be completed as it appears on your passport. If applying for J-2 visa for a spouse, contact SWAP for more information. Education and Experiences Complete your educational information in full, and list any prior J-1 visas you may have held including Summer Work and Travel or prior Internship visas. Emergency Contact Complete in Full. Host Site Information Complete all of the fields of the contact information of your internship host site. WISE will use this information to contact your host site to complete the DS-7002. Permanent and Current Address Complete as appropriate. Family in the US If you have family members in the United State please provide the complete information required for all members. It’s important to be honest and complete. Essay Questions Answer all of the essay questions with as much detail as possible. Answers should not be more than 250 words and in complete sentences (no bullet points please). A resume will also be required with your application, please review WISE’s resume requirements and make alterations as necessary. Please note, Canadian passport holders are NOT required to make or attend an appointment at a US Consulate to apply for a J-1 visa, and additional processing time is not required. As this application is for all international applicants and not just Canadians, please be aware that references to the consulate appointment are not applicable to you. Training and Internship Program Agreement Print in your name and read through all of WISE Foundations terms and conditions. Remember to sign and date that you understand and agree to abide by the terms and conditions on page 5. Optional Automobile Operator Insurance In addition to the travel insurance that is included in the program fee, if you will be driving or intend to drive your vehicle to the US, you may be interested in extending the insurance to cover you while in the US. This is optional, but if you are interested please fill out as appropriate. Liability Agreement and Medical Release Agreement Please read through both agreements carefully and print your name, sign and date each. Verification of Student Status or Graduation This information expands on the basic information provided on page one of your application. If you are currently enrolled in and pursuing studies a school representative (registrar, academic advisor, placement officer, counsellor or department head) from your academic institution. This form should be completed in full, in English, and must be either stamped with the school stamp or seal. If you have graduated, please complete the first section of the form and provide a copy of your diploma with your application. Medical Statement The medical statement must be printed and taken to your doctor to be completed. This form must be completed in full and in English. Interview Form Please leave blank, once submitted, SWAP will be in contact to arrange an interview with you in order to complete your application. Reference Form This form is required to be completed by a current or former employer or school instructor or administrator. This is a professional reference, and cannot be completed by a relative or friend. Current letters of recommendation (if less than a year old) from a school or employer may be substituted. SWAP APPLICATION CONDITIONS (document found on www.swap.ca ) This is a separate document. Don’t forget to sign it! REGISTRATION FEE (download the appropriate form on www.swap.ca) Please calculate your registration fee depending on your total stay in the US, from the date of departure to the U.S. until the date of return to Canada. Beginning a new month (even one day!) will be calculated as an entire month to your registration fee. In addition to your completed Application, SWAP Application Conditions and Registration Fee forms, please submit : A clear photocopy of the picture page of your Passport A current resume (review the resume requirements on page 3 of the application) A copy of an offer letter from your host site signed by you and the host site representative. SUBMITTING YOUR APPLICATION Processing time is 4-6 weeks from the time the USA Programs Coordinator receives your correctly completed application. Please note that additional processing time is needed if a site visit is required. For general inquiries please contact the USA Internship Coordinator with any questions or concerns at [email protected] (please put Internship/PCT in the email subject line) Insurance Coverage Your registration includes coverage by medical, liability, and travel insurance during your time in the US (you will receive your insurance confirmation in your pre-departure information). It is important that your provincial GHIP does NOT expire during your absence from Canada as this will cause your existing GHIP to be void for as long as three months after you return home, leaving you without medical coverage in this time while you wait to be re-instated. It is your responsibility to maintain the status of your provincial GHIP. NOTIFYING PROVINCIAL GOVERNMENT HEALTH INSURANCE PLAN (GHIP) Often people travelling outside of Canada for longer than 3 months are not covered by their provincial government health insurance plans (GHIP). This varies among provinces. Please ensure that you contact your provincial GHIP office. Many provincial GHIP plans require you notify them about leaving Canada for longer than 3 months in order to qualify for coverage. Note: If you are asked the purpose of your trip, remember that SWAP is a program of cultural exchange and that “work” is incidental to the cultural experience. Some provincial GHIP plans will not cover citizens leaving Canada for the sole purpose of work; in other GHIP plans, the reason “work” could be the easiest way to get your coverage extended. So please check the regulations of your provincial health insurance plan.
