Outline how to fill Registration Documents
of Measurement Facilities

1. Required forms for registration
Documents used for application shall be written either in Japanese or English. Attachments
written in other languages shall be translated either in Japanese or English to aid examination of
applications.
1.1 New registration
First, review Normative Annex 2, “REGULATION FOR REGISTRATION OF MEASUREMENT
FACILITIES” (V-5), and determine under which Article of the following you are going to register your
measurement facility:
(1) Article 3 of V-5, (2) Article 14 of V-5, or (3) Article 15 of V-5
Note: If you newly register your measurement facility under Article 14 of V-5 the facility shall have been filed
to FCC. If you newly register your measurement facility under Article 15 of V-5 the facility shall have been
accredited by one of the Laboratory Accreditation Bodies specified in Article 15 of Normative Annex 2.
1.1.1 New registration under Article 3 of V-5
(1) Forms required for newly registering a facility for measuring radiated disturbance:
Forms 101, and the documents specified in item 5 of Form 101.
(2) Forms required for newly registering a facility for measuring conducted disturbance at Mains ports:
Forms 102A, and the documents specified in item 5 of Form 102A.
(3) Forms required for newly registering a facility for measuring conducted disturbance at
Telecommunication ports: Forms 102B, and the documents specified in item 5 of Form 102B.
(4) Forms required for newly registering a facility for measuring radiated disturbance above 1GHz:
Forms 101G, and the documents specified in item 5 of Form 101G
1.1.2 New registration under Article 14 of V-5
(1) Form required for newly registering a facility for measuring radiated disturbance:
Form 151 and the documents specified in item 5 of Form 151.
(2) Form required for newly registering a facility for measuring conducted disturbance at Mains ports:
Form 152A and the documents specified in item 5 of Form 152A.
1.1.3 New registration under Article 15 of V-5
(1) Form required for newly registering a facility for measuring radiated disturbance:
Form 211 and the documents specified in item 4 of Form 211.
Note: Required submission is a single sheet per location where situated are multiple accredited
measurement facilities under the same accreditation number. Registration will be done collectively with a
single registration number per location.

1.2 Renewal of Registration
First, review Normative Annex 2, “RULES FOR REGISTRATION OF MEASUREMENT FACILITIES”
(V-5), and determine under which Article of the following you are going to register your measurement
facility:
(1) Article 3 of V-5, (2) Article 14 of V-5, or (3) Article 15 of V-5
Note 1: If your measurement facility is already registered under one Article, and you renew the
registration under another Article, the effective period of your registration, and other conditions,
will be different.
Note 2: If you renew your registration of measurement facility under Article 14 of V-5 the
the facility shall have been filed to FCC. If you renew your registration of measurement
facility under Article 15 of V-5 the facility shall have been accredited by one of the Laboratory
Accreditation Bodies specified in Article 15 of Normative Annex 2.
1.2.1 Renewal of registration under Article 3 of V-5
(1) Forms required for renewing the registration of a facility for measuring radiated disturbance:
Forms 107, and the documents specified in item 5 of Form Form 107.
(2) Forms required for renewing the registration of a facility for measuring conducted disturbance at Mains
ports:
Forms 108A, and the documents specified in item 5 of Form 108A.
(3) Forms required for renewing the registration of a facility for measuring conducted disturbance at
Telecommunication ports:
Forms 108B, and the documents specified in item 6 of Form 108B.
(4) Forms required for renewing the registration of a facility for measuring radiated disturbance:
Forms 107G, and the documents specified in item 5 of Form 107G.
1.2.2 Renewal of registration under Article 14 of V-5
(1) Forms required for renewing the registration of a facility for measuring radiated disturbance:
Form 157 and the documents specified in item 5 of Form 157
(2) Forms required for renewing the registration of a facility for measuring conducted disturbance at Mains
ports:
Form 158A and the documents specified in item 5 of Form 158A.
1.2.3 Renewal of registration under Article 15 of V-5
(1) Forms required for renewing the registration of a facility for measuring radiated disturbance:
Form 211 and the documents specified in item 4 of Form 211.

measurement facilities accredited under the same accreditation number. Registration will be

2. Notes for filling out forms
2.1 Forms 101, 101G, 102A, and 102B
(1) Item 1 (a): Enter the name of the company that has been registered with VCCI, and the member number
that VCCI has assigned to the company.
(2) Item 1 (b): Enter the name of the person responsible for the registration. The bill for the examination
fee, and the certificate of registration, will be sent to that person. If you want these
documents to be sent to any other person, provide the name and address of that person on an
attached sheet.
(3) Item 1 (c): Enter the name of a contact person, in case VCCI needs to ask any questions about the
application.
(4) Item 2 (a): If you have more than one measurement facility, enter the unique name of the facility you are
applying to register.
Example: (1) XXX Laboratory, site No. 1. (2) YYY Plant, site A.
(5) Item 3: For Form 101, enter the name of the standard that was applied to the measurement of NSA.
Refer to sections 5.3.3.6 (1) and 5.3.3.6 (2) of the Technical Requirements (V-3).
(6) Item 3: For Form 101G, enter the method of site validation whichever one

2.2 Forms 103, 103G, 104, and 104G
(1) Form 103, Item 2 (a): If the shape of the metal ground plane is not rectangular, enter “Refer to the
attached drawing,” and attach a drawing that clearly shows the shape of the metal ground
plane.
(2) Item 3 (c): If you use an embedded turntable that complies with section 5.3.3.4 of the Technical
Requirement (V-3), enter the structure of the electrical (radio-frequency) connection
between the circumference of the turntable and the metal ground plane. If you use
a contact-type connection, enter the structure of the contact point and the number of
contact points (for example, “Metal brush, 20 points”). Attach drawings or photos to
show the structure.
2.3 Forms 105A, 105B
(1) Item 2 (a): Check the reference plane you use, and write the size of the plane.
(2) Item 2 (b): If you use a movable metal reference plane rather than a metal wall plane or floor plane in
the shielded room, enter the method of connection to the grounded metal plane.
(3) Item 2 (c): Write the method of installation of the horizontal metal reference plane
(4) Item 3 (b): Enter the units for the length and the gauge of the wire. Enter the cross section of the gauge.
2.4 Form 106A,106B and 106G
(1) As a rule, when you apply to concurrently register the measurement facilities for radiated disturbance,
and conducted disturbance at the Mains ports, you should fill out Form 106A for each measurement
facility. However, if identical measurement equipment is used in each facility, you can fill out one
Form 106A describing all measurement facilities and equipment, and attach a copy of that Form 106A to
the application for each measurement facility. In case of conducted disturbance at the Telecommunication
ports, use Form 106B, and radiated disturbance above 1GHz, use Form 101G.
(2) Equipment to measure EMI: Enter the name of the equipment used to measure the disturbances from the
EUT. Use the symbols described in Note 3.

2.5 Forms 107, 107G,108A, and 108B
(1) Item 1 (a): Enter the name of the company that has been registered with VCCI, and the member
number that VCCI has assigned to the company.
(2) Item 1 (b): Enter the name of the person responsible for the registration. The bill for the examination
fee, and the certificate of registration, will be sent to that person. If you want these
documents to be sent to any other person, provide the name and address of that person on
an attached sheet.
(3) Item 1 (c): Enter the name of a contact person, in case VCCI needs to ask any questions about the
application.
(4) Item 2 (a): If you have more than one measurement facility, enter the unique name of the facility you
are applying to register.
Example: (1) XXX Laboratory, site No. 1. (2) YYY Plant, site A.
(5) Item 3: For Form 107, enter the name of the standard that was applied to the measurement of NSA.
Refer to sections 5.3.3.6 (1) and 5.3.3.6 (2) of the Technical Requirements (V-3/2009.04).
(6) Item 3: For Form 107G, enter the measurement method for site validation whichever one.
(7) Examination fee (above the cell for “VCCI use only”) : Choose the way you prefer to pay the
examination fee.
2.6 Forms 111
(1) Item 1 (a): If the site was too small to allow the use of a full-length antenna, and you therefore used a
shortened dipole antenna in measuring the site attenuation, check “Applied.” If you did not
use a shortened dipole antenna, check “Not applied.”
(2) Item 1 (b): If you, by yourself, calibrated the antenna used in measuring NSA, describe the method
used in the calibration. If you apply section 5.3.3.6 of the Technical requirement
(V-3) or section 5.3.3.6 (1) of (V-3), you must calibrate the antenna in accordance with
Normative Annex 1-3 2.1.1 , “Calibration of Dipole Antenna Used in Evaluation of Measuring
Test Facility (Normalized Site Attenuation measurement).”
Examples of the description:
・ The method of antenna calibration with the standard dipole antenna
・ The standard site method in accordance with ANSI C63.5
・ The replacement method referenced to the calibrated antenna
・ The standard electric field method
・ Other method (describe the calibration method)
(3) Item 1 (c): If you apply section 5.3.3.6(2) of the Technical Requirement(V-3) or section 5.3.3.6 (2) of the
(V-3) in measuring NSA, and the conditions of the measurement are as follows, describe d1
and d2, defined below:
You measure the NSA using an antenna that has a length in the measurement axis (such as
a bi-conical or bi-log antenna), at a measurement distance of 3 m, and in the frequency
range from 30 MHz to 1 GHz.
d1: The distance between those elements of the two antennas (refer to Fig. 1) which are
used at the lowest frequency.
d2: The distance between those elements of the two antennas (refer to Fig. 1) which are
used at the highest frequency.

(4) Item 1 (d): For the semi anechoic chamber, describe the requirements for the broadband antenna used
in measuring the site attenuation by the frequency-sweeping method.

2.7 Forms 109A, 109B, 110A, and 110B
(1) Name of the applied standard: Enter the standard you selected in item 3 of Form 101 or 107.
(2) Calibration conditions/Antenna height: If you apply VCCI V-3 5.3.3.6(1) to your measurement, specify
which height, 2 m or 3 m, was used in the free space. For the conditions of the antenna calibration, refer to
Normative Annex 1-3 2.1.1 ,”Calibration of Dipole Antenna Used in Evaluation of Measuring Test
Facility(Normalized Site Attenuation measurement)”.
(3) Equipment calibration laboratory (A)/Calibration date: At “A,” write the name of the laboratory that
calibrated the measurement equipment.
(4) Correction factor, ΔNSA: If you apply VCCI V-3 5.3.3.6(1) to your measurement, choose an appropriate
correction factor from Table 3 to 11 in Appendix 1 of Technical Requirements (V-3), depending on
whether the antenna used in measuring the NSA was a tuned dipole antenna or a half-wave dipole antenna.
For the types of antenna, refer to the list of the following table. When you apply VCCI V-3 5.3.3.6(2) to
your measurement, the correction factor shall be zero (0) for all measurements.

2.8 Forms 111G
(1) Item 2 : For the height of test volume, write the distances between floor and top of test volume and
between floor and bottom of test volume
(1) Item 5 : Enter the worst result of Svswr measurement.
(2) Item 6 : Attach the document indicated by item 6.
2.9 Forms 151, 152A
(1) Item 1 (a): Enter the name of the company that has been registered with VCCI, and the member number
that VCCI has assigned to the company.
(2) Item 1 (b): Enter the name of the person responsible for the registration. The bill for the examination fee,
and the certificate of registration, will be sent to that person. If you want these documents to
be sent to any other person, provide the name and address of that person on an attached sheet.
(3) Item 1 (c): Enter the name of a contact person, in case VCCI needs to ask any questions about the
application.
(4) Item 2 (a): If you have more than one measurement facility, enter the unique name of the facility you are
applying to register.
Example: (1) XXX Laboratory, site No. 1. (2) YYY Plant, site A.
(5) Item 3: Enter FCC registration (file) number, and its expiry date. If the registration date is undefined
during requesting new or revised for FCC registration, leave blank the date and registration
number in newly request. When defined, contact VCCI soon.
(6) Item 3: The standard that applies to Forms 151 and 152A is ANSI C63.4.2003 American National
Standard for Method of Measurement of Radio Noise Emissions from Low Voltage Electrical and
Electronic Equipment in the Range 9 kHz to 40 GHz
(7) Examination fee (above the cell for “VCCI use only”) : Choose the way you prefer to pay the
examination fee.
2.10 Forms 157, 158A
(1) Item 1 (a): Enter the name of the company that has been registered with VCCI, and the member
number that VCCI has assigned to the company.
(2) Item 1 (b): Enter the name of the person responsible for the registration. The bill for the examination
fee, and the certificate of registration, will be sent to that person. If you want these
documents to be sent to any other person, provide the name and address of that person on
an attached sheet.
(3) Item 1 (c): Enter the name of a contact person, in case VCCI needs to ask any questions about the
application.
(4) Item 2 (a): If you have more than one measurement facility, enter the unique name of the facility you
are applying to register.
Example: (1) XXX Laboratory, site No. 1. (2) YYY Plant, site A.
(5) Item 3: Enter FCC registration (file) number, and its expiry date. If the registration date is undefined
during requesting new or revised for FCC registration, leave blank the date and registration

2.11 Forms 211
(1) Item 1 (a): Enter the name of the company that has been registered with VCCI, and the member that
VCCI has assigned to the company.
(2) Item 1 (b): Enter the name of the person responsible for the registration..
(3) Item 1 (c): Enter the name of a contact person, in case VCCI needs to ask any questions about the
application..
(4) Item 1 (c): Item 2 (a): Enter a unique name of the location (capacitated by a single accreditation scope)
Example: (1) XXX Establishment (2) EMC Center, YYY Plant
(5) Item 2 (b) (c) (d): Enter the accreditation number from the certificate issued by the laboratory
accreditation body by which accreditation is granted , and its expiry date
(6) Item 2 (e) (f) Enter the registered accreditation number and its expiry date. However, this is not
necessary for the first time registration.
3. How to organize the application documents (for other than Article 15-effected applications)
If “n” facilities to be registered refer to a single common document, make “n” copies of the
document and attach each to the individual facility application. Either that or unmistakably indicate
what application carries the common document to be referenced.
Include “information necessary to evaluate documents with” in attached data.

3.1 Application documents for measurement facilities should be filed in such a way that they are sorted
by the kind of facility to be registered. Each application document should be preceded by table of
content and marshaled in the order indicated in the example below. Bundles of papers should be
clipped together so the order of papers is not disarrayed. A sample of table of content is available
with Form 100A.
The following example is a package of application documents required for the registration of
a semi-anechoic chamber for the measurement of radiated disturbance.

As to the frequency, fill out 35 kinds prescribed in Form 109A (no need for more than 35 kinds)

(9) Data of cable loss of the coaxial cable as attached to Form 101 which comes with model name of the
cable (e.g. RG-214, not to be confused with assembly name), cable length, date of measurement etc.:

(10) Block diagram of EMI measurement system (clarify relationship with loss of coaxial cable)
(11) Drawings and photographs attached to Form 104 (which clearly show electrical coupling structure
between the periphery of the turntable and metal ground plane, if the turntable is a built-in type.
(12) Data on frequency sweeping attached to Form 111
Make data into a graph and fill in the measurement frequency step condition. This data should not be
confused with that for Form 109B. Refer to Form 111 for detail.
