Application Check List – Dorrance Continuing Education Scholarship Deadline: April 30th, 2014 ___________________________________________________ Applicant's Name _________________________ University Assigned ID I have read meet ALL of the following eligibility requirements:       Current recipient of the Dorrance Community Colleges Scholarship at South Mountain Community College. Completion of an Associate’s Degree, or an approved program of study. Minimum cumulative 2.75 GPA. Demonstrate financial need as exhibited by the FAFSA (www.fafsa.ed.gov). Admitted to an Arizona public university. Evidence of motivation and potential for successful transition to a four-year university. Complete your Dorrance scholarship application online by 5pm, but mail ALL* of the following together in the same envelope to be received by our office by April 30th, 2014: This Check List. Official transcript from South Mountain Community College and any other post-secondary institution you have attended in the previous three years. Please explain all withdrawals. Letter of acceptance from your College/Department at an Arizona public university. If you have not already done so: Complete the FAFSA application (without current financial information, your application will not be considered). File a Consortium Agreement if you are enrolled at more than one Maricopa Community College campus. Request a letter of recommendation from your SMCC academic advisor or a SMCC faculty member whose class you have taken. This will be submitted online by your recommender by May 5th, 2014. Complete and submit your online application.
