To receive your merchandise certificate, submit the following postmarked no later than January 31, 2011:
1. This Merchandise Certificate request form, filled out completely.
2. The original cash register receipt or invoice from your Great Planes, Hobbico or Flyzone model purchase.
The receipt or invoice must clearly show the retailer’s name and date of purchase.
Photocopies cannot be accepted. Refer to the Official Rules below.
3. The original proof-of-purchase from your Great Planes, Hobbico or Flyzone model box (see illustration).
No photocopies will be accepted.
