A SAMPLE YOUNG LIFE VOLUNTEER LEADER SURVEY by Frank Ivey In multiple club areas or areas with a large team of volunteer leaders, it is a useful practice to solicit feedback regularly. The following is a survey which my area has used to help gauge the health, attitudes and effectiveness of our volunteer leaders. It was a blind survey — we did not ask for the name of the leader filling it out — in order to help facilitate honest responses. It can be administered via mail or at an area-wide leadership meeting. This survey will be most effective if you: a) adapt the questions to the particular needs and situation of your area, b) use it once a year, c) set it up well by letting your leaders know in advance both why and when you are doing the survey, and d) share the results with your leaders and your committee.
