UNDERTAKING
1. Our College or Institution, ______________________________________________________
wishes to work as a Voluntary Help Center for the Online Admission Process for year 20142015.
2. We assure that while working as a Help Center our College or Institute shall engage welltrained staff and adequate infrastructure facilities as provided in the format. The College or
Institution shall not charge from the student for providing facilities as Cyber Space, print
outs or any other. The College or Institute shall abide by to all duties and functions assigned
from time to time by the Gujarat University Admission Committee (GUAC), Gujarat
University, Ahmedabad for Help Centers.
3. We agree that any expenses incurred on account of this shall be borne by our College or
Institution and any claims what so ever shall not be acceded to by the GUAC.

2.5. Duties and Functions
All the Help Centers (HC) are working under the Head of College or Institute, who is responsible
for smooth functioning of Help Center. One Nodal Officer not below the rank of Assistant
Professor shall be appointed at Help Center for overall coordination of Help Center. One
Network Administrator is required to take care of issues related to requirement of computers,
printers and internet connections etc.
Following are the different functions to be performed at Help Center during admission process.
A) Registration Process:
1) The Help Center will function from 11.00 to 17.00 hours.
2) Provide necessary guidelines to the candidates during all stages of admission processes.
3) Allow the candidates to use internet facility at Help Center for registration and
subsequent processes.
4) Provide facility for printing of registration form free of cost.
5) Verify the registration form duly signed by the candidate as well as his guardian and its
enclosures with original documents for confirmation of registration. If found OK, take
print out of Registration Acknowledgement Slip in two copies. Issue one copy to the
candidate with signature of Nodal Officer with seal of Help Center. Retain second copy
of Registration Form along with all self-attested copies of certificates, second copy of
Registration Acknowledgement Slip and keep it with the Help Center.
6) Daily record of registration etc should be kept on hand by the Nodal Officer.
B) Choice Filling and Mock Rounds:
Allow the candidates to use internet facility at Help Center for choice filling as well as to see
the results of mock rounds.
C) Allotment of admission and reporting at Help Center:
1) Check the admission Information Letter and paid Bank Challan of the candidate.
2) Verify all the original documents of the candidate including the following:
a) SSC Mark sheet
b) HSC Mark sheet
c) School Leaving Certificate/Transfer Certificate
d) Caste Certificate (If applicable) (issued by Authorities of Gujarat State only)
e) Parishistha-‘K’ in Gujarati (Non-Creamy Layer Certificate, in case of SEBC Category)
duly signed by Officer not below the rank of Taluka Development Officer (TDO).
f) Income Certificate issued after 31st March 2014 showing income less than 4.5 Lakhs
in the name of father only. (If father is not surviving then in the name of Mother).
The Issuing Authorities are either ‘Mamlatdar’ or ‘Taluka Development Officer
(TDO)’ or Collector (Jan Seva Kendra) at the time of registration for admission. No
other certificate/document will be considered valid (e.g. Income Tax Return copy,
Certificate issued by ‘Sarpanch’ of Grampanchayat etc.)
3) Write allotted program, college or institute name at the top left corner on back side of
Std. 12th original mark sheet with a black permanent marker pen for the actual
admission round-01 and with a blue permanent marker pen for actual admission round02.
4) Then after issue the “Admission Letter’ duly signed by the Help Center Nodal Officer
with seal of Help Center.
5) In case of candidate who are not having any of the original document/certificate ask
them to report at the GUAC, Ahmedabad to pay additional Rs. 5000/- as a deposit for
getting “Admission Letter”.
6) In case of any discrepancy the Help Center Nodal Officer shall contact the GUAC.
D) Important Instructions:
1) There should be separate arrangement for Cyber Space and Verification Space. Use
Token System in case of heavy rush.
2) To change password or edit registration details of registered candidate, candidate
should fill up “Request Form for Editing Registration Details” completely (mobile number
is must) along with self-attested copies of required documents and testimonials. Help
Center Official shall send such candidate along with self-attested copies of required
documents and testimonials to GUAC to make corrections. Such changes shall be
reflected in candidate’s account after two days.
3) Allot separate computers to accept registration forms, to create master data and
operations related with Help Center module from Cyber Space. So frequent login/logout
problems can be eliminated.
4) Summary of Help Center wise Registration Acknowledgement Slips and deficiency
generated and admission confirmed would be available for reference.
5) Following authorities are competent authorities to issue following certificates.
a. Income Certificate:
i. Mamalatdar/Nayab Mamalatdar
ii. Taluka Development Officer (TDO)
iii. Certificate with round seal of Mamalatdar Office issued by Jan Seva
Kendra Officer
b. Disability Certificate: Civil Surgeon of Government Hospital
c. Caste Certificate:
i. Scheduled Caste (SC):
1. For City: District Backward Class Welfare Officer
2. For Rural: Social Welfare Officer, Panchayat
3. Taluka Development Officer (TDO)
4. Mamalatdar/Nayab Mamalatdar
5. Competent Authority authorized by Government of Gujarat
ii. Scheduled Tribe (ST):
1. District Vigilance Officer
