טרם נטילת התרופה חובה לעבור בדיקה רפואית מקיפה, על מנת לוודא שהטיפול מתאים למטופל ושלא קיימות בעיות קרדיווסקולריות (לב וכלי דם) ,שכן פעילות מינית כרוכה ברמה מסויימת של מאמץ ללב. פורמט עלון זה נקבע ע"י משרד הבריאות ותוכנו נבדק ואושר על ידו עלון לצרכן לפי תקנות הרוקחים (תכשירים) התשמ"ו 1986 - תרופה זו חייבת במרשם רופא קרא בעיון את העלון עד סופו בטרם תשתמש בתרופה: סילדנאפיל טבע® 25 טבליות מצופות ® סילדנאפיל טבע 50 טבליות מצופות מ"ג מ"ג ® סילדנאפיל טבע 100 טבליות מצופות הרכב: כל טבליה מצופה מכילה: מ"ג Sildenafil (as Citrate) 25 mg Sildenafil (as Citrate) 50 mg ildenafil (as Citrate) 100 mg S מרכיבים בלתי פעילים: Microcrystalline cellulose, Dibasic calcium phosphate anhydrous, Croscarmellose sodium, Magnesium stearate, Polyvinyl alcohol, Titanium dioxide, Macrogol/PEG 3350, Talc. כל טבליה של 25מ"ג מכילה 0.3-0.5 :מ"ג נתרן כל טבליה של 50מ"ג מכילה 0.65-0.95 :מ"ג נתרן כל טבליה של 100מ"ג מכילה 1.3-1.9 :מ"ג נתרן קבוצה תרפויטית: מעכב את פעולת האנזים .PDE5 פעילות רפואית: לטיפול באין אונות ( )Erectile dysfunctionאצל גברים. מתי אין להשתמש בתכשיר? • אין להשתמש בתרופה זו אם ידועה רגישות לאחד ממרכיביה. • התרופה אינה מיועדת לנשים. • אין לתת תרופה זו למטופלים מתחת לגיל 18שנים. • אין להשתמש בתרופה אם הנך נוטל באופן רציף או לסרוגין ניטראטים מכל סוג שהוא (תרופות המשמשות להקלת תעוקת לב -כאב בחזה) ,כולל תרופה המשחררת ניטריק אוקסיד ()NO מאחר ושילוב של סילדנאפיל עם ניטראטים עלול לגרום לירידה מסוכנת בלחץ הדם. • אין להשתמש בתרופה אם הנך סובל מבעיה חמורה בכבד או בלב. • אין להשתמש בתרופה אם סבלת לאחרונה מהתקף לב או משבץ מוחי או אם הנך סובל מלחץ דם נמוך. • אין להשתמש בתרופה אם הנך סובל ממחלת עיניים תורשתית נדירה כגון .Retinitis pigmentosa • אין להשתמש בתרופה אם סבלת בעבר מאובדן ראייה מסוג NAION ).)Non arteritic anterior ischemic optic neuropathy אין להשתמש בתרופה מבלי להיוועץ ברופא לפני התחלת הטיפול • אם הנך סובל או סבלת בעבר מליקוי בתיפקוד: • הלב. • הכבד (כגון שחמת). • מערכת העיכול -כגון אולקוס או דימומים (כגון המופיליה). • מערכת הדם -כגון בעיות של קרישה ,אנמיה (,)Sickle cell anemia סרטן של מח העצמות ,לוקמיה. • אם הנך סובל מעיוות בצורת איבר המין או מחלה מסוג Peyronie's ( diseaseמחלה הגורמת לעיוות איבר המין בזמן הזיקפה). • אם יש לך בעיות הקשורות לכליות או אי ספיקה כלייתית חמורה. • אם יש לך לחץ דם גבוה (לחץ דם גבוה מ.)170/110 - • מטופלים מעל גיל 65שנה יטופלו במינון התחלתי נמוך. • אם הנך סובל מירידה פתאומית או מאובדן כללי של הראייה (ראה סעיף "תופעות המחייבות התייחסות מיוחדת"). איך תשפיע התרופה על חיי היום יום שלך? השימוש בתרופה זו עלול לפגום בראייה או לגרום לסחרחורת ,על כן מחייב זהירות בנהיגה ברכב ,הפעלת מכונות מסוכנות ובכל פעילות המחייבת עירנות. אזהרות: במידה ונטלת סילדנאפיל טבע® והנך זקוק לטיפול רפואי כלשהו, חובה לדווח מיידית לצוות הרפואי כי נטלת תרופה זו על מנת למנוע אפשרות שיינתן לך טיפול בניטראטים או במרחיבי כלי דם אחרים. • אם הנך רגיש למזון כלשהו או לתרופה כלשהי ,עליך להודיע על כך לרופא לפני נטילת התרופה. • במידה והנך סובל מאובדן או מירידה פתאומית בראייה ,יש להפסיק ליטול סילדנאפיל טבע® ולפנות לרופא מיד. • בזמן הטיפול בסילדנאפיל טבע® אין להשתמש בתרופות אחרות לטיפול באין אונות. • במידה ומופיע כאב בחזה בזמן קיום יחסי מין או בסיומם ,אין ליטול ניטראטים לטיפול בכאב ,פנה לרופא מיד לקבלת טיפול רפואי. תגובות בין-תרופתיות: אם הנך נוטל תרופה נוספת ,או אם גמרת זה עתה הטיפול בתרופה אחרת, כולל תרופות הנמכרות ללא מרשם רופא או תוספי תזונה וויטמינים ,עליך לדווח לרופא המטפל כדי למנוע סיכונים או אי-יעילות הנובעים מתגובות בין-תרופתיות ,במיוחד לגבי תרופות מהקבוצות הבאות: • תרופות אחרות לטיפול באין אונות • ניטראטים או תרופות המשחררות ניטריק אוקסיד (אמיל ניטריט) - תרופות אלו משמשות להקלה בתעוקת לב (אנגינה) SILD CTAB PL SH 100714 • קטוקונזול (נגד פטריות) • תרופות לטיפול באיידס (כגון :סקוינויר ,ריטונויר) • סימטידין (לכיב קיבה) • אריתרומיצין (אנטיביוטיקה) • דוקסזוצין (לטיפול ביתר לחץ דם) • תרופות מקבוצת חוסמי אלפא (לטיפול ביתר לחץ דם או הגדלה של בלוטת הערמונית) תופעות לוואי: בנוסף לפעילות הרצויה של התרופה בזמן השימוש בה עלולות להופיע השפעות לוואי כגון: תופעות לוואי שכיחות: • כאב ראש • אדמומיות בפנים • קשיי עיכול • הפרעות בראייה (כולל טשטוש בראייה או רגישות לאור) • סחרחורת • גודש באף תופעות לוואי בלתי שכיחות: • הקאות ,בחילה • פריחה • כאב או גירוי בעיניים ,עיניים אדומות ,דימום בעין ,הפרעות בראייה (ראייה כפולה) • דופק מהיר או בלתי רגיל • כאבי שרירים • ישנוניות • ירידה בתחושת המישוש • צלצולים באוזניים ,סחרחורת • יובש בפה • עייפות • כאב בחזה תופעות לוואי נדירות: • לחץ דם נמוך או גבוה • עילפון • שבץ מוחי • דימום מהאף תופעות לוואי נוספות: • התקף אפילפסיה או הישנות של התקפי אפילפסיה אם התופעות הללו מתמשכות ו/או מטרידות יש לפנות לרופא. תופעות המחייבות התייחסות מיוחדת: • במידה והנך חש בכאבים בחזה בזמן קיום יחסי המין או לאחריהם: יש להיות במצב ישיבה ולהירגע ,אין ליטול ניטראטים להקלת כאבי החזה ויש לפנות לרופא מיד. • ירידה פתאומית בראייה או אובדן ראייה פתאומי בעין אחת או בשתי העיניים ,ירידה פתאומית בשמיעה או איבוד שמיעה כללי :הפסק הטיפול ופנה לרופא מיד. • תגובה אלרגית חריפה הכוללת קשיי נשימה ,צפצופים ,סחרחורת, נפיחות של העפעפיים ,הפנים ,השפתיים או הגרון :יש לפנות לרופא מיד. • זיקפה ממושכת ,הנמשכת יותר מארבע שעות ,ולעיתים מלווה בכאב: פנה לרופא מיד. בכל מקרה שבו הנך מרגיש תופעות לוואי שלא צויינו בעלון זה ,או אם חל שינוי בהרגשתך הכללית ,עליך להתייעץ עם הרופא מיד. מינון מינון מקובל בהעדר הוראה אחרת מהרופא: טבליה אחת כשעה לפני קיום יחסי מין. אין ליטול יותר מטבליה אחת ביום .אין לעבור על המנה המומלצת. מינון לפי הוראות הרופא בלבד. תרופה זו אינה מיועדת למטופלים מתחת לגיל 18שנים. שים לב: התרופה יעילה רק כאשר יש גירוי מיני .הזמן לתחילת השפעת התרופה שונה ממטופל למטופל ,בדרך כלל השפעת התרופה תחל חצי שעה עד שעה לאחר נטילתה. אופן השימוש: אין ללעוס! יש לבלוע הטבליה עם מים. ניתן ליטול סילדנאפיל לפני או אחרי הארוחה. מומלץ לא להרבות בשתיית אלכוהול ולא לאכול ארוחה כבדה לפני נטילת התרופה. מנע הרעלה! תרופה זו וכל תרופה אחרת יש לשמור במקום סגור מחוץ להישג ידם של ילדים ו/או תינוקות ועל ידי כך תמנע הרעלה. אם נטלת מנת יתר או אם בטעות בלע ילד מן התרופה ,פנה מיד לחדר מיון של בית חולים ,והבא אריזת התרופה איתך. אל תגרום להקאה ללא הוראה מפורשת מהרופא! תרופה זו נרשמה לטיפול במחלתך; בחולה אחר ,היא עלולה להזיק. אל תיתן תרופה זו לקרוביך ,שכניך או מכריך. אין ליטול תרופות בחושך! בדוק התווית והמנה בכל פעם שהנך נוטל תרופה .הרכב משקפיים אם הנך זקוק להם. החסנה: לאחסן במקום יבש מתחת ל.25ºC - גם לפי תנאי האריזה/ההחסנה המומלצים ,תרופות נשמרות לתקופה מוגבלת בלבד .נא לשים לב לתאריך התפוגה של התכשיר! בכל מקרה של ספק ,עליך להיוועץ ברוקח שסיפק לך את התרופה. אין לאחסן תרופות שונות באותה אריזה. מס' רישום התרופה: 25מ"ג147.84.33479.00/01 : 50מ"ג147.85.33480.00/01 : 100מ"ג147.86.33474.00/01 : יצרן :טבע תעשיות פרמצבטיות בע"מ ,ת"ד ,3190פתח-תקוה. בעל הרישום :סלומון ,לוין ואלשטיין בע"מ ,ת.ד 3696 .פתח-תקוה. Therapeutic activity: For treatment of erectile dysfunction in men. When should thE preparation not be used? •Do not use this medicine if there is a known sensitivity to any of its ingredients. •The medicine is not intended for women. •Do not give this medicine to patients below the age of 18 years. •Do not use the medicine if you continuously or periodically take any kind of nitrates (medicines used to relieve angina pectoris – chest pain), including a medicine that releases nitric oxide (NO), since a combination of Sildenafil with nitrates may lead to a dangerous decrease in blood pressure. •Do not use the medicine if you are suffering from a severe liver or heart problem. •Do not use the medicine if you recently had a heart attack or stroke or if you are suffering from low blood pressure. •Do not use the medicine if you are suffering from a rare hereditary eye disease, such as retinitis pigmentosa. •Do not use the medicine if you have suffered in the past from loss of vision of the NAION (non arteritic anterior ischemic optic neuropathy) type. Do not use this medicine without consulting a doctor before starting treatment •If you are suffering, or have suffered in the past, from impaired function of: •The heart. •The liver (e.g., cirrhosis). •The digestive system – such as ulcer or bleeding (e.g., hemophilia). •The blood system – such as coagulation problems, anemia (sickle cell anemia), cancer of the bone marrow, leukemia. •If you suffer from a deformity of the penis or a disease of the Peyronie's disease type (a disease that causes a deformity of the penis during erection). •If you have problems relating to the kidneys or severe kidney failure. •If you have high blood pressure (blood pressure higher than 170/110). •Patients above the age of 65 years will be treated with a low initial dosage. •If you are suffering from a sudden decrease or general loss of vision (see "Effects that require special attention" section). How will this medicine affect your daily life? Use of this medicine may impair vision or cause dizziness and therefore, caution must be exercised when driving a car, operating dangerous machinery and when engaging in any activity that requires alertness. Warnings: If you took Sildenafil Teva® and need any medical treatment, you must immediately inform the medical staff that you took this medicine, in order to avoid the possibility of being given nitrates or other vasodilators. •If you are sensitive to any food or medicine, inform the doctor before taking the medicine. •If you suffer from a loss or a sudden decrease of vision, stop taking Sildenafil Teva® and refer to a doctor immediately. •During the course of treatment with Sildenafil Teva®, do not use other medicines for the treatment of erectile dysfunction. •If chest pain occurs during or at the end of intercourse, do not take nitrates to treat the pain; refer to a doctor immediately to receive medical treatment. Drug interactions: If you are taking another medicine, or if you have just finished treatment with another medicine, including non-prescription medicines or nutritional supplements and vitamins, inform the SILD CTAB PL SH 100714 attending doctor in order to prevent hazards or lack of efficacy arising from drug interactions. This is especially important for medicines belonging to the following groups: •Other medicines for treatment of erectile dysfunction •Nitrates or medicines that release nitritc oxide (amyl nitrite) – these medicines are used to relieve angina pectoris •Ketoconazole (antifungal) •Medicines to treat AIDS (e.g., saquinavir, ritonavir) •Cimetidine (for gastric ulcer) •Erythromycin (antibiotic) •Doxazosin (to treat hypertension) •Medicines from the alpha blocker group (to treat hypertension or prostate enlargement) Side effects: In addition to the desired effect of the medicine, side effects may occur during the course of taking this medicine, such as: Common side effects: •Headache •Facial redness •Indigestion •Vision disturbances (including blurred vision or sensitivity to light) •Dizziness •Stuffed nose Uncommon side effects: •Vomiting, nausea •Rash •Eye pain or irritation, red eyes, bleeding in the eye, vision disturbances (double vision) •Rapid or irregular pulse •Muscle pains •Sleepiness •Decreased sense of touch •Ringing in the ears, vertigo •Dry mouth •Tiredness •Chest pain Rare side effects: •Low or high blood pressure •Fainting •Stroke •Nosebleed Additional side effects: •Epileptic attack or recurrence of epileptic attacks If these effects persist and/or are bothersome, refer to a doctor. Effects that require special attention: •If you experience chest pain during or after intercourse: relax in a sitting position, do not take nitrates to relieve chest pain and refer to a doctor immediately. •Sudden decrease of vision or sudden loss of vision in one eye or both eyes, sudden decrease of hearing or general loss of hearing: stop the treatment and refer to a doctor immediately. •A severe allergic reaction which includes breathing difficulties, wheezing, dizziness, swelling of the eyelids, face, lips or throat: refer to a doctor immediately. •Prolonged erection, lasting more than four hours, and sometimes accompanied by pain: refer to a doctor immediately. In the event that you experience side effects not mentioned in this leaflet, or if there is a change in your general health, consult the doctor immediately. Dosage The usual dosage unless instructed otherwise by the doctor: One tablet about one hour before intercourse. Do not take more than one tablet a day. Do not exceed the recommended dose. The dosage is according to the doctor's instructions only. This medicine is not intended for patients below the age of 18 years. Attention: The medicine is only effective when there is sexual stimulation. The time to the start of the effect of the medicine varies between patients; the effect of the medicine will usually begin a half hour to one hour after taking it. Directions for use: Do not chew! Swallow the tablet with water. Sildenafil can be taken before or after a meal. You are advised not to drink excessive amounts of alcohol and not to eat a heavy meal before taking this medicine. Avoid poisoning! This medicine and any other medicine must be kept in a safe place out of the reach of children and/or infants to avoid poisoning. If you took an overdose, or if a child accidentally swallowed the medicine, immediately refer to a hospital emergency room and bring the package of this medicine with you. Do not induce vomiting without explicit instruction from the doctor! This medicine has been prescribed to treat your ailment; in another patient, it may cause harm. Do not give this medicine to your relatives, neighbors or acquaintances. Do not take medicines in the dark! Check the label and the dose each time you take medicine. Wear glasses if you need them. Storage: Store in a dry place, below 25°C. Even when stored as per the recommended packaging/storage conditions, medicines may be kept for a limited period only. Please note the expiry date of the preparation! In any case of doubt, consult the pharmacist who dispensed this medicine to you. Do not store different medicines in the same package.
