FAILURE TO SHOW CORRECT AVERAGE WILL DISQUALIFY THE BOWLER AND ALL PRIZE MONEY WILL BE FORFEITED. To participate in this tournament each bowler must have a current USBC membership card and be a member of the Northern Utah USBC Association. Dues of $20.00 will be assessed each bowler not in possession of a USBC membership card and a card will be issued. Dues of $9.00 will be assessed from each non-association USBC cardholder and an association membership will be issued. Non-members living within the jurisdiction of this association who are not members of another association may enter by paying an Affiliate Membership Fee of $20.00. (USBC Rule 300c) Only members of the Northern Utah USBC Association are eligible to enter. 2. No entry fee will be returned. 3. All prize fees will be returned 100% within each event, at a ratio of at least one prize for each eight entries or major fraction thereof. 4. Scratch Team Event, Scratch Singles & Scratch Doubles Event winners receive a winner’s plaque. The Scratch All-Events winner will receive a plaque and have their name engraved on the Ben Day Trophy and the Trophy will be displayed in the winner’s home bowling center. You do not have to enter the optional Scratch Events to be eligible to win the scratch all-events trophy. 5. Bowlers may participate in the four person team event as a member of more than one team, only under the following conditions: a. The bowler must be a bona fide member of a league from within the Northern Utah USBC Association. b. The bowler must have at least 12 games as of November 1, 2014, or 21 games or more from the previous season within the Northern Utah USBC Association, or a verifiable USBC average. c. No more than two players can compete together more than once in the four-person team event. d. When a bowler enters team play more than once, the first series bowled shall count towards the all-events score. e. The captains will be responsible for making any average corrections. USBC Rule 319a-2, the 10-pin rule, is waived. 6. DRESS CODE – TAKEN FROM THE USBC NATIONAL CHAMPIONSHIPS -- NO EXCEPTIONS: Participants must wear a shirt and casual slacks, trousers or jeans. Traditional collared shirts, mock collared shirts and collarless bowling jerseys are all permitted. T-shirts will not be permitted. Women may wear Capri pants provided the length is below the knees. During participation in any event, bowlers are NOT permitted to wear: hats, tank tops, halter tops, bare midriffs, spaghetti strap tops, etc., workout attire, bib overalls, sweats, shorts, skirts, skorts, culottes or zubas “pants with elastic cuffs”, (except for medical reasons). All clothing must be neat, clean and in good condition (No torn jeans). Designated tournament staff members will enforce the dress code and bowlers will be required to change if attire is deemed inappropriate by tournament management or designee. They will make judgment calls relative to dress code questions or concerns, while any questionable items or special circumstances will be addressed by the Manager on Duty. Tournament management or designee decisions will be final. PRIDE AWARD: In order to reward teams dressed in a “Team Uniform” NUUSBC has created the local “Pride Award.” To qualify for the award, all four members of a team must wear matching shirts and pants of the same color (no jeans) when they bowl their team event. Qualifying teams will receive special “pride” pins. 7. Bowlers may participate only once in the doubles and singles events. All bowlers must participate in both regular doubles event and regular singles event. Bowlers who for health reasons cannot bowl singles and doubles (6 games) at one squad time may request separate squad times for these events. Request for this health reason must be submitted with entry form. Only those entering and paying the all-events fee or the optional scratch events fees can qualify for cash awards in the allevents or optional scratch events. You do not have to pay the all-events fee to win the all-events trophies. Senior bowlers may enter all three all-events. 8. Handicap will be based on 90% of the difference between the bowler’s highest established 2013-14 Northern Utah USBC Association yearbook average and 240. If no previous 2013-14 Northern Utah USBC Association yearbook average, highest current NUUSBCA average of 12 games or more as of November 1, 2014 or a verifiable USBC average shall be used. A bowler holding membership in two or more USBC Associations must use their highest 2013-14 USBC established book average. All others bowl scratch. Except that bowlers with previous NUUSBCA yearbook averages may apply for re-rated average based on the previous NUUSBCA yearbook average. Responsibility for application for re-rated average rests with the bowler, and the application must be made one week before the entry deadline. The Tournament Manager will not verify averages unless a bowler cashes in an event. 9. It shall be each bowler's responsibility to verify the accuracy of their average, whether submitted by the bowler or the team captain or others. Failure to use the proper average shall disqualify score if submitted average is lower than actual average thereby resulting in a lower classification or more handicap. Prize winnings shall be based on the submitted average if it is higher than the actual average. Corrections of averages can be made up to the completion of the first game. The following must be submitted if necessary. Tournament prize winnings of $600.00 or more in any one event in the past twelve months must be submitted to the tournament manager (USBC Rule 319-C, D and E). USBC Playing Rules Book will be used. 10. Any rescheduling after the regular schedule has been posted must be made by team captains and/or the individuals. The tournament manager shall be notified of any changes. Unmarried grade and high school students who have not attained the age of eighteen (18) must have written consent of their parents or guardian to participate in this tournament (USBC Rule 13). Each group or individual whose name appears on this entry blank does hereby agree to abide by the tournament rules and regulations of the Northern Utah USBC Association together with the playing rules of the United States Bowling Congress. The Tournament Chairperson will settle any disputes and the Tournament Chairperson’s decision is final.
