Program
Full year program commences September 8, 2014 and ends June 6, 2015. Each class is guaranteed a minimum of 32 classes plus dress rehearsal & recital.
Studio will be closed for the following holidays: Thanksgiving Weekend, Halloween, Remembrance Day, Christmas (2 Weeks), Teachers Convention, Family Day, Spring
Break (1 Week), Good Friday & Victoria Day Weekend.

Tuition
Registration fee: $25 (non-refundable)
Forms of Payment:
! Full Year – cash, debit card, credit card or cheque payable to Dance On Edge Inc.
! Monthly – 9 post-dated cheques payable to Dance On Edge Inc., dated for the 1st of each month (Sept–May). June payment is added to Sept. cheque and is nonrefundable.
Conditions:
! Payment ensures spot in class and must be received prior to class commencement.
! All dance fees and services are subject to 5% GST.
! Post-dated cheques must be in the office by October 15, 2014 or a service fee of $40.00 will be applied to your account.
! All RETURNED cheques are subject to a service charge of $25.00 (amount subject to change by bank) and must be honoured in cash within 7 days.
! A service charge of $ 20.00 on all late fees after 30 days. Outstanding balances will accumulate a $20.00 service charge every 30 days until paid in full.
! Missed classes are non-refundable; make up classes may be requested but are subject to availability.
! Dance On Edge Inc. reserves the right to cancel classes, full refund given.
! Tuition fees do not include fees for exams, competitions, extra classes, dance attire, costumes, candid or formal photographs or recital tickets.
! For an additional fee, lessons for exams and competition work will be available. Enrolment in classes does not guarantee entry into exams or competition program.

Withdrawals
• Student withdrawals must be received in writing one month prior to withdrawal. You are then responsible for one month payment and the remaining cheques will be
returned. Notice must be received prior to the 1st of the month before it can be processed for the following month. Partial monthly tuition will not be refunded. No refunds
will be given after February 1st.
Photography/Videos
From time to time the school will be retaining the services of a professional photographer to take pictures of the students. I consent to my
child having their picture taken while participating in Dance On Edge Inc. programs, including class/group pictures. Pictures are optional
to purchase, but proofs will be placed online on a password protected website for convenient customer viewing/ordering.
I consent to Dance On Edge Inc. to take/use any photos/videos of student’s participation in dance programs and related activities for
display, promotional, publicity and/or advertising purposes.

Dance On Edge Inc. has the right to refuse or to suspend any individual who infringes on any school policy or rules. Please refer to student handbook for details.
Student placement in class is at the discretion of Dance On Edge Inc. Dancers are placed in a level that is most suitable to their ability. Placement could change after
classes have begun if Dance On Edge Inc does not feel the level is suitable for dancer’s proper development.
Mandatory dress code must be adhered to. This includes proper fitting dance shoes. Students may be asked to sit out if proper dress code is not adhered to.
No chewing gum in class or peanut products within the studio environment.

Please call if a student is going to be absent from class.
Dance On Edge Inc. is not responsible for any items lost or stolen. Do not leave valuables unattended. Bulky items will not be permitted into the classroom space.
If a problem arises regarding a child please call the office to set up an appointment with their teacher. Problems will not be discussed during class time.
Waiver
I/We the undersigned hereby release and discharge Dance On Edge Inc., its directors, officers, shareholders, employees and contractors from any and all actions, causes of actions
for damages, losses or injuries of any nature or kind which may be sustained by the student in consequence of enrolment in the school. I/We agree to be bound by the rules contained
in the registration form. Dance On Edge Inc. shall not be held responsible for any loss or theft of any personal possessions whatsoever. I/We authorize Dance On Edge Inc. to seek
medical service in case of serious injury or illness if I am unable to be contacted. I further agree to accept financial responsibility that are in excess of my benefits plan that I may have.
Students will not be admitted to class without the signature on this form and full payment in the office. September and June Fees are Non-Refundable.
