Important Facts
• No refund or adjustment will be made for a change of choice or spacing
after a plate has been ordered or if the request form is incorrectly
completed.
• The Secretary of State’s office is authorized to recall a plate determined
to be obscene or offensive.
• All request forms should indicate whether you want the Secretary of
State to assign one of your choices with the next available number if your
exact request is unavailable.
• If you wish to submit more than three requests, attach additional
selections to the request form.
• Numbers are required to precede letters on personalized B-truck, RV and
RT plates.
• If you want a space between letters, please indicate with a slash mark. An
automatic space is required between letters and numbers. Spacing
varies with the type of plates requested. Proper completion of the form is

Vanity/Personalized Plates
• Money must accompany request forms on all first-time registrations.
• If your request is not available, random-number plates will be issued.
Additional fees are required to replace your random-number plates with
vanity or personalized plates.
• The dealership where you purchase your vehicle may apply for vanity or
personalized plates on your behalf for a first-time registration.
• A leased vehicle is eligible for vanity or personalized plates.

plates allow you to be creative, say
something special or help support an
important cause.
Vanity and personalized plates are multi-year and may be displayed on passenger vehicles, B-trucks weighing 8,000 pounds or less, motorcycles,
recreational vehicles and recreational trailers.
To request standard vanity or personalized plates, please submit the Vanity & Personalized License Plates Request Form on the back of this brochure.
Availability of plate combinations cannot be given over the phone. If your plate request is available, you will receive an application to be completed
and returned along with the appropriate fees within 45 days. DO NOT send money unless you are applying for title and first-time registration.
Most specialty plates require special fees in addition to the vanity or personalized fee. For more information on specialty and

Vanity and personalize messages are available on standard
passenger, B-truck, motorcycle, RV and RT plates as well as on a
