Payment of bills lists dated 10/20/14 through 10/24/14 in the amount of $. b. 2011 Bond Issue/065 Reserve Account/Assessment Office Renovation Project: Requesting approval for payment of an invoice received from Contract Hardware and Supply Inc., in the amount of $290.00 for 1 door and passage set. c. NBIS Bridge Inspection: Requesting approval for payment of Invoice #31 received from Stiffler McGraw in the total amount of $3,350.63 for inspection of the following Blair County Bridges: Bridge #17, Rebecca Furnace Road over Clover Creek, $1,483.55 and Bridge #18, Furry Mill Bridge over Clover Creek, $1,867.08. Expenses will be paid out of the Act 44 Reserve Account (No. 059) and are 80% reimbursable. d. Blair County Bridge #43, Tyrone 12th Street: Requesting approval for payment of Construction Estimate #7, received from Wen-Brooke Contracting, Inc., in the amount of $13,025.12. Expenses are 100% reimbursable, and will be paid out of the Act 44 Reserve Account (No. 059). e. Terminations: Demetrius L. Davis and Marcus G. Hepborn, Fill-In Correction Officer’s, Prison, ineligible for rehire, effective 8/15/14 and 10/20/14 respectively. 7. PROCLAMATION (Blair County Children, Youth and Families) Adoption of a Proclamation proclaiming the month of November as “National Adoption Awareness Month” in the County of Blair. 8. STAFF REPORTS & SPECIAL BUSINESS A. Blair County Human Resources: a. Requesting approval of a collective bargaining agreement between the County of Blair and SEIU Healthcare Pennsylvania, CTW, CLC (Building and Grounds, Highway) effective January 1, 2014 through and including December 31, 2016. b. Requesting approval of a settlement agreement and release between the County of Blair and SEIU Healthcare Pennsylvania, CTW, CLC, and its members for unpaid, unused, accrued vacation, sick, holiday and personal time accrued by bargaining unit members prior to the June 1, 2013 transfer date of Valley View Home to Reliant Valley View, LLC. B. Blair County Department of Social Services: Requesting approval of seven (7) FY 2014/2015 Renewal Contracts between the County of Blair and the following: 1. Blair County Community Action Program for Representative Payee, Housing Assistance Program – Maximum contract amount: $60,000 2. Blair Senior Services for Guardianship Protection Service, Durable Power of Attorney, Domiciliary Care, Human Service Development Fund, Housing Assistance Program – Maximum contract amount: $200,000 3. Contact Altoona for Peer Outreach and Mentoring Team and Reassurance Calls, Human Service Development Fund – Maximum contract amount: $100,000 4. Home Nursing Agency for Outpatient Services, Administrative Case Management Resource Coordination, Blended Case Management, Housing Support, Social Rehabilitation, Psychiatric Rehabilitation, Family Support, Partial Hospitalization, Outpatient Psychiatric, Community Residential and Peer Specialist Services – Maximum contract amount: $1,600,000 5. NAMI Blair County Affiliate for Family Support Services, and Peer Recovery Conference – Maximum contract amount: $31,500 6. Skills of Central PA for Employment Services, Facility Based Vocational Rehabilitation, and, Housing Support– Maximum contract amount: $405,590 7. UPMC Altoona for Administrative Case Management/BSU, Consultation and Education, Student Assistance Program, Resource Coordination, Emergency Services, Crisis Intervention, Family-Based Mental Health, Outpatient Psychiatric and Children’s Psychosocial Rehabilitation Services – Maximum contract amount: $1,525,000 C. Blair County Planning Commission: Requesting approval to enter into Sub-Recipient Agreements between the County of Blair, Blair County Planning Commission and the following organizations in order that they may receive funding under the County of Blair’s Pennsylvania Housing Affordability and Rehabilitation Enhancement Fund (PHARE) Grant Agreement, administered by the Pennsylvania Housing Finance Agency (PHFA): o Blair County Community Action Agency $ o Blair Senior Services $ o Catholic Charities $ D. Blair County Veterans’ Affairs Office: Requesting approval of an additional thirty-six month (36) Lease Agreement to the Costars Master Lease Agreement dated August 15, 2011 between the County of Blair and Word Processing Services, Inc., (WPS) for the lease of one (1) Savin 5200S copier, printer, scanner, fax unit for the Veterans’ Affairs Office, in the amount of $83.03 per month for 3,000 copies per month with an overage cost of $0.01300 per page. E. Blair County Purchasing: Requesting approval of an addendum change to the existing agreement between the County of Blair and Burgmeier’s Hauling Inc., for the exchange of one (1) four (4) cubic yard rear-load dumpster to one (1) eight (8) cubic yard rear-load dumpster with side door, at the Blair County Highway Department, at no additional charge. 9.
