Dear Members of the AFP Upper Mississippi Valley Chapter:
As you know, National Philanthropy Day® is the day we recognize and pay tribute to the contributions that
philanthropy and philanthropists have made to our great region and nation.
On behalf of the AFP Upper Mississippi Valley Chapter and National Philanthropy Day, we invite nominations for
the following awards: Outstanding Philanthropist, Outstanding Volunteer Fundraiser, Outstanding Philanthropic
Organization, Outstanding Professional Fundraiser, and Outstanding Youth in Philanthropy.
In the past, we have been honored to be able to recognize individuals and organizations that have made the
commitment to support philanthropy while keeping the spirit of civic betterment alive and strong. This year our
organization again looks forward to taking an inventory of those who have made a difference in our respective
organizations and communities. Your participation in this process is vital to the accomplishment of our goal—
to recognize exceptional individuals and organizations that have helped to advance philanthropy.
Any community member may now nominate someone for an award. Please feel free to forward this information
to people in the community. Enclosed is a brief description of each award, followed by questions and guidelines to
assist you in completing a nomination. These questions—and your narrative responses to them—will serve as the
primary basis of the application and the board’s award selections. Please answer each of the questions in detail for

2014 AWARDS GUIDELINES
The Association of Fundraising Professionals (AFP) sponsors an annual awards program to recognize
outstanding achievement by individual, foundation, and corporate philanthropists; fundraising volunteers; and
professional fundraising executives.
In professional fundraising, such individuals and organizations form a vital partnership that philanthropy must
have to maintain its contribution to humankind. It is right and proper, therefore that the Upper Mississippi
Valley AFP Chapter honors those who hold high ethical standards and best exemplify commitment, dedication,
and talent toward helping society.
These awards are given in recognition to those individuals and organizations whose life-time or long-term
achievements have made a significant impact in our region.
For each award, please review and follow these guidelines:
1. If submitting a new nomination, please answer each of the questions noted completely and thoroughly.
YOUR ANSWERS WILL SERVE AS THE PRIMARY BASIS FOR AWARD SELECTIONS!
2. Your Nomination should include:
a) Cover page (pages 1 and 2)
b) Nomination Form – not to exceed three pages
c) Up to three letters of endorsement – each letter should not exceed 2 pages. Resumes, annual reports, or
personal vitae are not accepted as part of the application.

AFP Outstanding Youth in Philanthropy

In 2010, the Upper Mississippi Valley Chapter of AFP introduced this new award—Outstanding Youth in Philanthropy—to
recognize a K-12 student who has demonstrated outstanding commitment to the community through helping to generate
direct financial support, development of charitable programs, and leadership in philanthropy.
Your nomination for this award should include the following:
Description of what the nominee did, including goals, process, budget, and other people involved in the process (adult
leaders or adviser) and of the time, talent, and/or treasure the youth gave for the common good and the results that
were achieved.
Description of the impact the nominee/project had on the community, including numbers of people involved,
organizations helped, etc. Please include the names and contact information of individuals at the organizations who
benefited from the nominee’s work.
Long-term (1-2 years) impact of the nominee’s work. Will the project/s be ongoing? Has the nominee considered the
next steps? Is s/he working on other projects?

AFP Outstanding Professional Fundraiser
Note: Answers to the following questions should not exceed three pages.
This award is designed for an outstanding individual fundraising professional and AFP member who works directly for a
nonprofit, charitable organization and practices his/her profession in an exemplary manner. Generally, consultants or other
individuals not working directly for a charity are ineligible for this award. Nominees must have a minimum of 10 years of
experience as a professional fundraiser working directly for a nonprofit organization (years as a consultant will not be
considered). Your nomination should include the following:
Description of career achievements, length and tenure of fundraising background, experience and results
Evidence of quality of leadership that is effective, creative, and stimulating, and evidence that the individual acts as a
team player
Active membership in AFP and other professional organizations, and evidence that the individual practices and
promotes the AFP Code of Ethical Principles and Standards of Professional Practice, the Donor Bill of Rights, and
similar principles.
Description of the nominee’s commitment to continuing professional development (e.g., CFRE/ACFRE certification,
conference participation, publication, and/or professional education) and demonstration of sharing knowledge and
expertise through presentations, mentoring, publications
