Resume/Curriculum Vitae; 2. Authorization To Contact References; 3. University Transcripts and Diplomas (i.e., B.A., B.Ed.); 4. Pastoral Reference www.rccdsb.edu.on.ca/files/2010/11/priest_ref.pdf; 5. Two current Professional Reference Letters (experienced Teachers); OR 6. Practice Teaching Reports (new grads only); 7. QECO Statement of Evaluation – Program 5 (if available); 8. Ontario College of Teachers Certificate of Qualification; OR 9. OCT Interim Certificate (if certification is pending, please indicate); 10.
