A. Coroner’s Office
1. Approval of the Second Amendment to the October 20, 2010, professional
services agreement with NMS Labs, Willow Grove, PA, to provide forensic
toxicology testing services to the Coroner’s office. Term of the agreement is
January 1, 2015, through December 31, 2017, and includes a three percent
increase on current fees for routine toxicology panels and storage for one year
on samples submitted for analysis.
B. Emergency Services
1. Approval of a quote from Butler Health System to provide physical exams for
the Haz-Mat team members, as required under federal law, for $338 per exam,
less $50 per exam if an x-ray is not required. 100% reimbursed through the
HMEP grant.
C. Facilities & Operations
1. Approval of an agreement with Armstrong Business Division to provide Internet
service and static IP addresses for the new Cranberry Twp. MDJ office.
Monthly fee is $74.95 with an installation charge of $79.95; term is 36 months
with automatic renewal for successive one-month terms.
2. Approval of an agreement with Consolidated Communications for telephone
lines and service at the new Cranberry Twp. MDJ office. Monthly fee is
$210.85; term is 24 months, month-to-month afterward.

D. Human Services – Area Agency on Aging
1. Approval of a contract with the Center for Community Resources to disburse
$64,904 in PA Aging Disability Resource Coordination (ADRC) Grant funds.
Term is July 1, 2014, through June 30, 2015. No County funds are involved.
E. Human Services – Community Action and Development
1. Approval to amend the 2013/2014 Supportive Services for Veteran Families
(SSVF) contract with the Center for Community Resources, to increase the total
contract amount from $127,000 to not exceed $129,000. Term remains the
same, October 1, 2013, to September 30, 2014.
2. Approval to submit the interim Addendum Letter Agreement for The Emergency
Food Assistance Program (TEFAP) to the PA Department of Agriculture to
incorporate additional program operating requirements into the current 20122016 contract.
F. Human Services – Drug and Alcohol Program
1. Approval of a contract with Sojourner House for long-term inpatient treatment
for adult women, pregnant women, and women with children at $236.76/day,
effective December 1, 2014, through June 30, 2015.
G. Human Services
1. Approval of a contract with Catholic Charities, Diocese of Pittsburgh, for the
Home Again Butler County program in the amount of $168,118. Term of the
contract is December 1, 2014, through November 30, 2015. Fully funded by the
U.S. Department of Housing and Urban Development.
H. Parks & Recreation
1. Approval to enter into a commission agreement with Louise Peterson for a lifesized bronze dog statue for the future dog park at Alameda Park, subject to
Solicitor review.
2. Approval to enter into a consulting service agreement with Pashek Associates
for the preparation of construction documents, bidding, and construction
observation for the future dog park at Alameda Park, subject to Solicitor review.
3. Approval to enter into an agreement for professional engineering services with
Arcadis for the preparation of construction documents, bidding, and
construction observation for a restroom facility at Winfield Township Community
Park, subject to Solicitor review.
4. Approval to enter into an agreement with Senate Engineering for engineering
services for the preparation of construction documents for a playground area in
Petrolia Borough Park, subject to Solicitor review.
I. Prison
1. Approval of Amendment 6 to the May 18, 2009, agreement with Trinity
Services I, LLC, to manage the inmate food service operation and facilities at
$1.296 per meal, an increase of $.034, effective January 1, 2015.

J. Commissioners
1. Adoption of the following Resolutions related to the 2015 Budget:
a. Resolution #2014-___ – Adoption of the 2015 Budget
b. Resolution #2014-___ – Real Estate Tax
c. Resolution #2014-___ – Real Estate (Debt) Tax
d. Resolution #2014-___ – Reaffirmation of GASB Statement 54
2. Approval to hire Armand Cingolani, Esq., as special counsel at a rate of
$135/hour.
2. Approval to reappoint the following individuals to the Farm Land Preservation
Board for three-year terms, expiring December 2017:
a. Roy Andrews, Butler
b. Robert Burr, Prospect
c. William Duncan, Saxonburg
3. Ratification of the approval of an agreement with the PA Housing Finance
Agency (PHFA) for a PHARE grant in the amount of $400,000 for two different
projects: $300,000 to be allocated for the Owner Occupied Rehabilitation
Program (matched with $75,000 from the County’s Act 137 monies), and
$100,000 allocated to the First-Time Homebuyer Assistance Program (matched
with $100,000 from the County’s Act 137 monies).
4. Approval of letters of support to the PA Housing Finance Agency (PHFA) and
the Housing Authority of the County of Butler for the Creekside Apartments
project in Jackson Township, along with a commitment of $300,000 from the
County’s Act 137 monies, contingent upon the successful awarding of tax
credits by PHFA.
5. Approval of a quote from Midwest Employers Casualty Co. through Raymond
James Financial Services for renewal of the County’s Excess Workers’
Compensation & Employers’ Liability Policy at a rate of .2251 per $100 payroll.
Term of the agreement is January 1, 2015, through December 31, 2015.
