Ratification of the acceptance of a proposal for snow removal at the Cranberry MDJ site. 2. Ratification of the acceptance of a proposal for the provision of floor mats at the Cranberry MDJ site. 3. Approval of an addendum to the MHY Family Services contract for the provision of drug testing services to the Adult Probation Office. B. Emergency Services 1. Ratification of authorization to apply for the Emergency Management Performance Grant (EMPG). 2. Ratification of authorization to apply for the 2015-2016 wireless grant and for the Chairman to certify that the funds will be used according to Chapter 53, Emergency Telephone Service, of Title 35 of the PA Consolidated Statutes. 3. Approval to revise a previous agreement with Consolidated Communications to provide long-distance services to three outgoing 911 lines in addition to providing the current outgoing business lines. This new agreement will roll the new numbers into the current agreement at no additional charge. 1 C. Facilities & Operations 1. Approval to renew an agreement with Simplex Grinnell for fire alarm inspection, fire pump test and inspection, hood cleaning, and kitchen hood inspection at the Prison, in the amount of $20,865. The renewal is for three years, April 1, 2015, through March 31, 2018. No price change from previous agreement. 2. Ratification of the approval of a service order from Consolidated Communications to add a line to hook into the fire panel at the Cranberry MDJ site in the amount of $26.55 per month. 3. Approval of Change Order #GC-005 for installation of tapered roof insulation and replacement of the EPDM roof membrane on the Government Center Annex in the amount of $50,504.53. The work has already been completed, per Change Order Directive CCD-GC-001, dated November 25, 2014. 4. Approval of a five-year contract with Vogel Disposal to provide trash service for the Cranberry MDJ office at $35.00 per month for a 96-gal container, subject to Solicitor’s review. D. Prison 1. Approval to renew an agreement with Butler County Community College to provide training for Prison personnel in the total amount of $6,800. E. Assessment 1. Ratification of the approval of a fee quote from Thomas R. Frederick of Frederick & Co. for appraisal services related to upcoming assessment appeals. 2. Approval of a Payment in Lieu of Taxation Agreement by and between Mars Area School District, Adams Township, County of Butler, and Mars Holdings, Inc. for Tax Parcels Nos. 010-3F66-58F, a senior-living apartment complex currently known as Overbrook Pointe. F. Bridge Program 1. Report on the bid opening for the abutment system for the Iron City Bridge project. G. Mapping Department 1. Approval to renew a software maintenance agreement with ESRI for 2015 in the amount of $12,378 with authorization for the Chairman to sign. This agreement covers the GIS software maintenance for Mapping, Emergency Services, Conservation District, and Information Technologies as follows: a. Mapping $5,228 b. Emergency Services $1,450 c. Conservation District $700 d. Information Technologies $5,000 H. Parks & Recreation 1. Approval of the following reappointments to the Parks & Recreation Advisory Board: a. Diane Ziggas, Region 4 – term expires December 31, 2017. b. Judy Snyder, At-Large Representative – term expires December 31, 2017. 2 2. Approval of the following new appointments to the Parks & Recreation Advisory Board: a. Dustin Drew, Manager of Moraine State Park, At-Large Representative, replacing Jake Weiland – term expires December 31, 2017. b. Pete Geis, Recreation Director, Cranberry Twp., At-Large Representative – term expires December 31, 2017. c. Dave Zarnick, Butler Twp. Commissioner, filling the unexpired term of Region 1 – term expires December 31, 2015. I. Human Services – Children & Youth Agency 1. Approval of new contracts with Guardian Ad Litems Sue Lope and Ron Thomas. 2. Approval to reappointment Beth Gillan to the Children & Youth Agency Advisory Board for another three-year term ending January 31, 2018. J. Human Services – Drug & Alcohol Program 1. Approval to appoint Karianne Sarnese to the D&A Advisory Board for a threeyear term ending January 2018, replacing Denna Hays. K. Human Services – Area Agency on Aging 1. Approval of a license agreement with Saint Matthias. Term of the agreement is July 1, 2014, through June 30, 2015, at $875.00 per month. No County funds are involved. 2. Approval of a contract with the Nonprofit Development Corp. for snow removal and lawn services to individuals over the age of 60 in Butler County. Cost is $10,000. No County funds are involved. 3. Approval to advertise a Request for Proposals for remote monitoring services. L. Human Services – Community Action and Development 1. Ratification of the approval to electronically submit the Consolidated Capital Assistance Grant agreement to the PA Department of Transportation in the amount of $121,800. 2. Ratification of the approval of a contract with Lawrence County Community Action for CSBG discretionary funding for the veterans’ employment program. Term of the contract is January 1, 2015, through September 30, 2015, in the amount of $28,000. 3. Approval to amend the 2014 CSBG contract with Center for Community Resources to increase the contract amount by $3,689 for a new total of $60,153. 4. Approval to enter into a subcontract with the following agencies for the 2015 Community Service Block Grant funding. Term of all contracts is January 1, 2015, through December 31, 2015. a. Catholic Charities 54,000 b. Victim Outreach Intervention Center 32,000 c. Lighthouse 5,500 d. Butler County Children’s Center 11,000 e. CCR – UIPOC/VITA/Poverty Simulation 39,000 f. Butler County Blind Association – Lifeskills 5,500 g. Salvation Army – after-school program 10,000 h. Alliance for Nonprofit Resources 1,900 3 5. Ratification of the approval to post for 10 days a bid notice for the salvage sale of a totaled BART bus. M. Commissioners 1. Adoption of Resolution #2015-__ approving the Hospital Authority to issue bonds for refinancing all or a portion of the Butler Health System’s existing 2009 and 2010 bonds in order to achieve a lower interest rate. Adoption of this resolution does not constitute indebtedness nor pecuniary liability for the County. 2. Approval of an agreement with the Redevelopment Authority of the City of Butler for the administration of PA Housing Affordability and Rehabilitation Enhancement (PHARE) funds in the amount of $150,000. 3. Ratification of the Chairman’s approval of a Reconciliation Agreement with Sunnyview Realty LLC and Sunnyview Operating LLC. 4. Approval to appoint Neil Ashbaugh of Oberg Industries to serve on the TriCounty Workforce Investment Board. 5.
