The Creede School District 1 is seeking proposals for the following services
listed below and in the E-Rate 470 #104480001255566 form submitted on-line.
Category One
Local Phone Service -The District is seeking a two year contract with and
extension of three more years for five phone lines for voice and data usage.
Long Distance Phone Service - The District is seeking a two year contract
with and extension of three more years for five phone lines for long distance usage.
LAN - At least 1 GPBS for District is seeking a five year contract with a five year extension.
Internet Access – The district is seeking a five year contract for Internet access
For at least 30 MB or more of service with a flexible contract to increase services to meet future
needs. The District currently operates at 30Mbps and is looking to operate at a higher level. If
the proposer is able to offer higher levels of service, please provide those levels with an
associated service price per month for each respective level. The higher levels of service would
be used in the future to support a 1 to 1 initiative for the students, where anywhere from 80 –
100 students would be on a laptop or mobile device and connected to the internet at one time.
Please provide your recommendation as to what level of service might support a 1 to 1
initiative of this size, and if you would be willing to provide this level in the future.
Category Two
Wireless Lan - To serve at least 30+ Classrooms at the Elementary and Jr. Sr. High School
Classrooms.
The district will consider all inquiries. Decisions are made by the
Superintendent of School District with direction provided by the Technology
Committee and the Administrative team.
The district reserves the right to reject any/all proposals for the above listed
services. Funding for any services may be subject to budgetary constraints
from local and federal funding.

5. An implementation timeline proposal starting April/May, 2015.
6. Indicate how charges will be incurred as services are implemented.
7. Vendors must include 3 reference sites using your service 3 years or more. References from
