Read more… Safety, Quality, and Service- Strategic Priorities for 2015 To work most effectively as a system, it is important that physicians and clinical providers are aware of our objectives regarding Safety, Quality, and Service. Objectives, Benchmarks & Measurements: 1. Improve Patient Access As measured by a Transfer Center “acceptance rate” of 80 percent and a reduced LOS to meet or exceed budget in each facility. 2. Reduce Readmissions As measured by Premier’s unplanned readmission metric by 5 percent. 3. Improve Performance on Core Measures As measured by seven metric sets: AMI, HF, PN, SCIP, STK, VTE, and IMM. Scores based on performance through March 2015. Aim to reduce misses by 10 percent. 4. Reduce Hospital-Acquired Infections Each facility with two or fewer CAUTIs or CLABSIs will maintain or reduce the incidence. Facilities with more than two will reduce the incidence by 25 percent. 5. Improve Performance on HCAHPS Increase the overall rating of each hospital by 10 percentile points, as measured by the fourth quarter results.
