Summary Care Record
A Summary Care record
has been created for most
patients at the surgery.
A Summary Care Record is
a brief summary that consists of the patients: Name;
Address; DOB; NHS Number, allergy or allergic reaction information; acute and
repeat medication and any
discontinued medications in
the past six months.
A Summary Care Record is
set-up to enable Doctors/
Specialist Nurses outside of
your GP Surgery (and with
your permission) access,
should you require urgent/
emergency care. Having
access to allergy/
medication in theses
circumstances will enhance
the care that you will
receive.
Your approval will always
be sought whenever a
clinician requires access to
this information, (the only
occasion when a clinician
can override your permission for access would be if
you were too unwell to give
it), only clinicians: Doctors, Consultants; Nurse
Specialists can
access this information by
using a fully auditable
‘Smartcard’ that has been
issued to them, with rolebased permissions assigned
to them only to allow
access to the Summary
Care Record.

FRIENDS AND FAMILY
TEST
From December 2014 all GP
Surgeries in England will be
providing patients with the
ability to feedback their experience of services provided
by their GP Surgery.
Patients visiting the surgery
will be provided with a form
called the ‘Friends and Family
Test’.
The question ‘How likely are
you to recommend our GP
Practice to friends and family
if they needed similar care or
treatment?’ – is the same for
all surgeries. There will also
be another follow-up question
such as ‘Please tell us the reason for your answer, or suggest one thing we could do to
improve our service to patients’ - to allow patients to
elaborate if they wish to.
There is already a feedback
form on the website that is
very easily accessible for
those patients who can access
our website (see email address
overleaf).
The results of the survey will
be provided on a monthly basis on our website and will
also be available in the waiting room
