She brings with her a vast range of skills in terms of her previous years as a GP including tutoring for undergraduate medical education. Patient Access th From March 16 2015 the surgery will be able to send your prescription requests electronically to a Pharmacy that has been nominated by the patient for collection directly from them. Patients wishing to utilise the service can arrange this with a pharmacy of their choice (most pharmacies in the community will be enabled for this service or it can be organised at the Surgery). Online Appointment Booking Online appointment The way in which you booking will be available order your prescriptions to patients from mid from the Surgery will be February. changing soon. Online Medical Records From Mid February patients will also be able to access their Medical Records (Subject to identification confirmation), which will include all coded information – such as Diagnosis and Test Results. Further information regarding the above will be available soon by speaking to a member of the Reception Team or via the Website. If you are due to travel abroad and need advice Re: Vaccinations Please call into Surgery to complete a Travel Questionnaire Form. This will then be processed by one of the Nursing Team leaving information to pass to you regarding vaccinations required etc. Please telephone the surgery 48 hours after completing the form when the receptionist will arrange an appointment for vaccinations if they are required. Summary Care Record A Summary Care record has been created for most patients at the surgery. A Summary Care Record is a brief summary that consists of the patients: Name; Address; DOB; NHS Number, allergy or allergic reaction information; acute and repeat medication and any discontinued medications in the past six months. A Summary Care Record is set-up to enable Doctors/ Specialist Nurses outside of your GP Surgery (and with your permission) access, should you require urgent/ emergency care. Having access to allergy/ medication in theses circumstances will enhance the care that you will receive. Your approval will always be sought whenever a clinician requires access to this information, (the only occasion when a clinician can override your permission for access would be if you were too unwell to give it), only clinicians: Doctors, Consultants; Nurse Specialists can access this information by using a fully auditable ‘Smartcard’ that has been issued to them, with rolebased permissions assigned to them only to allow access to the Summary Care Record. February 2015 FRIENDS AND FAMILY TEST From December 2014 all GP Surgeries in England will be providing patients with the ability to feedback their experience of services provided by their GP Surgery. Patients visiting the surgery will be provided with a form called the ‘Friends and Family Test’. The question ‘How likely are you to recommend our GP Practice to friends and family if they needed similar care or treatment?’ – is the same for all surgeries. There will also be another follow-up question such as ‘Please tell us the reason for your answer, or suggest one thing we could do to improve our service to patients’ - to allow patients to elaborate if they wish to. There is already a feedback form on the website that is very easily accessible for those patients who can access our website (see email address overleaf). The results of the survey will be provided on a monthly basis on our website and will also be available in the waiting room PLEASE TELEPHONE IF YOU ARE UNABLE TO ATTEND AN APPOINTMENT THIS AVOIDS WASTED APPOINTMENTS WHICH CAN BE USED FOR PATIENTS WHO ARE TRYING TO BOOK TO SEE A DOCTOR OR ONE OF THE NURSES February 2015 Castlefields Surgery Newsletter Including New Patient Checks • Chronic Disease Management Clinic • Child Health Advice Patients can also use the web • Childhood Immunisite to order prescriptions. sations You will need to register on • Travel Advice/ line and then use your vaccinations Patient ID Number or DOB Including Yellow to identify yourself, and then Fever order on line. • Triage Clinic Out of Hours Service • Smoking Cessation NHS 111 • Family Planning Should you require urgent • Minor Ops medical attention outside of How to contact us: • Maternity Services normal surgery hours ———————————please telephone: Each GP provides an early By Phone: 01785 223012 Lisa Ferneyhough is our surgery at 7:00am – please 01785 223012 Clinical Support Worker you are then connected enquire with the receptionist By Post: Castlefields Surgery Lisa’s clinics are every automatically to access to book advance appointCastle Way advice or medical attention if morning from 9:00am – ments in one of these sesStafford 12:30pm for: required, from the Out of sions.
