I hereby certify that I have made due enquiries and that the answers to the following questions are as follows (please tick): Council Regulation (EC) No 1/2005 YES NO 1. Have you examined the animals within 48 hours prior to the intended time of departure? 2. Can you confirm that, in your opinion none of the animals are ill or injured, and that they appear fit and able to complete the intended journey without undue suffering? 3. As far as you can determine, have suitable provisions been made for the care of the animals during their journey, including feeding and watering if appropriate? Article 4 of Directive 92/65/EEC (Balai Directive) as amended 4. Do you have a TRACES-NDC authority from APHA Laboratories confirming that the holding of origin, from which the animals will be exported, has been registered in accordance with Article 4 of Directive 92/65/EEC? 2 YES 5. NO Have you received a satisfactory assurance from the owner/manager of the premises of origin that they will undertake to: (a) ensure that all the animals in their care at the premises of origin are suitably identified, and that Movement Records are maintained and made available for the competent authority to inspect. (b) inform CIT Exports, Carlisle of any suspicion of notifiable disease, or of any other disease for which a national control programme is in place? (c) comply with measures for the control of diseases for which national control programmes are in place? (d) export only animals which show no signs of disease, and which come from holdings and areas that are not subject to any ban on animal health grounds? (e) comply with national and EU regulations covering the welfare of the animals on their premises? Article 6 of Directive 92/65/EEC as amended 6. Are the animals for export suitably identified? See paragraph 4(e) of the NFG 7. Do you have a TRACES NDC Authority from CIT Exports Carlisleconfirming that the said animal(s): 8. (a) are not intended for slaughter under a programme for the eradication of an infectious disease; (b) come from a holding and an area which is not subject to animal health restrictions, including restrictions imposed for the control of either Classical or African Swine Fever; Do you have a Written Declaration from the owner/exporter stating that the animals to be exported have: (a) not been vaccinated against Foot and Mouth disease (b) been kept on the holding named at section I.12. of Part I of the ITAHC permanently since birth or for the past 30 days? 3 YES 9. NO In the case of Ruminants only Either: - Do you have Written Authority from APHA Laboratories confirming that the holding/herd at the address at section I.12. of Part I of the ITAHC is officially tuberculosis-free? Or: - can you confirm that on the premises named at section I.12. of Part I of the ITAHC no case of tuberculosis has been seen within the past 42 days? And - has each animal for export been tested for tuberculosis within 30 days of export with the intradermal comparative test using both avian and bovine tuberculins, with a negative result at the bovine injection site? Note: 10. Deer must be tested by an OV on panel 6(b) (deer) In the case of Ruminants only Either: - Do you have Written Authority from APHA Laboratories confirming that the holding/herd at the address at section I.12. of Part I of the ITAHC is officially brucellosis-free? Or: - Can you confirm that on the premises named at section I.12. of Part I of the ITAHC no case of brucellosis has been seen within the past 42 days? And - blood samples, taken from the animals to be exported within 30 days of export, have been sent to APHA Laboratories, Weybridge and submitted to the serum agglutination test for brucellosis with negative results in each case? (negative is less than 30 iu/ml) 11. In the case of Suidae only Either: - Do you have Written Authority from Animal Health and Veterinary Laboratories Agency confirming that the premises named at section I.12. of Part I of the ITAHC are an officially brucellosis-free holding? Or: 12. - Have blood samples been taken from the animals to be exported within 30 days of export and sent to APHA Laboratories, Weybridge and submitted to the Serum agglutination test for brucellosis with Negative results in each case (negative is less than 30 iu/ml), or the Buffered Brucella Antigen Test (BBAT) with negative results in each case?
