Form A 2015 Education Foundation Letter of Recommendation Request Form Applicant: Please give this form to the person writing your letter of recommendation. Complete the (*) sections of this form of prior to distributing to the evaluator. Only one letter of recommendation will be accepted. Additional letters of recommendation will be unopened and discarded. The purpose of the letter of recommendation is to provide the selection committee with third-‐ party documentation and validation of your background. They want to read the opinion of someone who is familiar with your background and knows you well. Suggested persons to write a letter of recommendation are: teacher or professor, employer, coach, or the director of a community service activity where you volunteered your time. You should select a person who cannot only write well, but write well about you. Family members may not write letters of recommendation.
