An alumna of Case Western Reserve’s
Frances Payne Bolton School of
Nursing, Florence Cellar enjoyed a 40year career at University Hospitals.
She established the Florence
Cellar Associate Professorship in
Gerontological Nursing— the country’s first chair in
gerontological nursing and the first fully endowed
professorship at the nursing school.

About the University Center on Aging and Health
Since 1978, the University Center on Aging and Health
(UCAH) has served as an interdisciplinary point at
Case Western Reserve University for building and
maintaining excellence in gerontological and geriatric

The 21st Florence Cellar Conference on Aging explores the transitions across the health care continuum.
It features the latest research findings in care transitions and care coordination including implications for
quality and safety. Successful models of transitional care and quality and safety using evidence-based
practice will be presented.
OBJECTIVES
• Compare current models that guide development
of programs to address care transitions for older
adults.
• Identify the evidence to optimize quality and
safety in care transitions.
• Describe factors related to successful care
transitions and care coordination
• Discuss communication barriers that obstruct
inter-professional and team collaboration in care
transitions and approaches to overcome them.

Marian S. Ware Professor of Gerontology, Director of New Courtland Center for Transitions and Health
Mary D. Naylor, PhD, RN, FAAN, is the Marian S. Ware Professor in Gerontology and Director of the New Courtland
Center for Transitions and Health at the University of Pennsylvania School of Nursing. Dr. Naylor is also the
National Program Director for the Robert Wood Johnson Foundation, Interdisciplinary Nursing Quality Research
Initiative, aimed at generating, disseminating, and translating research to understand how nurses contribute to
quality patient care. Dr. Naylor received her MSN and PhD from the University of Pennsylvania and her BS in nursing from Villanova
University. Dr. Naylor has taught both undergraduate and graduate level courses in nursing. Currently, she leads a seminar for the
Nature of Nursing Practice and Situating the Practice of Nursing.

Robert Applebaum, PhD
Director of the Ohio Long-Term Care Research Project, Professor, Department of Sociology and Gerontology,

Robert Applebaum is the Director of the Ohio Long-Term Care Research Project and a Professor at the Department
of Sociology and Gerontology. Applebaum is also serves as the Interim Associate Dean at the College of the Arts
and Science at Miami University. He is also a Scripps Research Fellow.
Gerri Lamb, PhD, RN, FAAN
Associate Professor, Arizona State University
Gerri Lamb started out as a home care nurse in lower Manhattan helping patients and families coordinate needed
services. She went on to lead seminal research into care coordination, including a 1990s Medicare demonstration
project that tested a fully capitated model for community-based care coordination. Recently, Lamb has been
working with the National Quality Forum on performance measures around care coordination. She is also project
co-director for INTERACT, a program that is helping reduce hospital admissions from skilled nursing facilities.

Senior Managing Partner, Improve Consulting and Training Group
Ellen Burts-Cooper, an adjunct professor at Weatherhead School of Management, Case Western Reserve University,
is the senior managing partner at Improve Consulting and Training Group, a firm that provides personal and
professional development training, coaching and consultation. Ellen and her team work with large corporations,
small companies, colleges/universities and nonprofit organizations to provide Lean Six Sigma Yellow and Green Belt
Certifications, project coaching, curriculum, development, training and project deployment. In addition, she provides business outcome
–focused leadership coaching as well as seminars and workshops on various topics from change leadership to project management.

Peter DeGolia, MD, CMD
Medical Director, McGregor PACE, University Hospitals Case Medical Center
DeGolia is a PACE physician/geriatrician at University Hospitals Case Medical Center. Recently, he was appointed
director of the Center for Geriatric Medicine and the medical director of the UH/Case House Calls Program. Dr.
DeGolia earned his medical degree at Rush Medical College in Chicago, completed his internship at University
Hospital and his residency and geriatric fellowship at MetroHealth Medical Center, both in Cleveland, Ohio. He is
board certified in family medicine, geriatric medicine, and hospice and palliative medicine. His areas of interest include education,
house call medicine, long-term care, and end-of-life care.
