Physicals Required for Junior High Sports
School District 54 requires that any student who participates in a team or intramural
sport (eg. cross-country, basketball, wrestling, volleyball, cheerleading, poms or track)
must have an approved physical on file in the health office prior to tryouts. The
physical must be administered by a physician or nurse practitioner and is valid for only
one year from the date of the exam.
Since sports physicals are valid for one year from the date of the exam, parents of
students who are interested in participating in any junior high sport are urged to
schedule a physical exam during the summer months. These appointments fill up
quickly! This will allow the student to participate in any sport throughout the school
year. It also coincides with the physicals required for high school admission.
If you have any questions, feel free to contact the nurse at the junior high school where
your child will be attending.
PLEASE MAKE A COPY OF THE PHYSICAL FOR YOUR OWN RECORDS
BEFORE TURNING IT INTO THE SCHOOL.
