Dear Parent(s)/Guardian(s):
A field trip has been scheduled for your child’s classroom. Please note that your child’s
participation in this educational experience is voluntary, not a required activity as part of
the District’s educational program/curriculum.
The District requires parents/guardians of minors to sign the attached “Permission Slip
and Save Harmless and Release of Liability” agreement. If a student is 18 years of age
or older, the student must sign the “Save Harmless and Release of Liability” agreement.
It is necessary that one of the parents or guardians sign the “Save Harmless and
Release of Liability agreement to enable your child to participate in this activity.
Thank you for your cooperation.
Sincerely,
Mary Kay Gallagher
