How to apply for a Co-op Work Permit and SIN Co-op Work Permits are required for: Co-op field placements, internships, externships and off-campus volunteer work whether paid or unpaid. Applicants are encouraged to complete the form electronically and validate the information to generate 2D barcodes before printing. To ensure you have the most recent application form, please use the link to the application form. All Student Work Permit applications must include the following: Completed Application to Change Conditions, Extend my Stay or Remain in Canada as a Worker (Form IMM 5710 Validated with Bar Codes) Completed Document Checklist (IMM 5583) Photocopies of passport “picture page” clearly showing the passport number, the dates of issue and expiry, name and date of birth, the stamp made by Canadian authorities of most recent entry into Canada and other marked pages. Original letter provided by the institution stating that your intended employment is an essential part of your program. Note: You can only begin working AFTER you receive a co-op work permit.
