How to Order Caris Target Now
A formalin-fixed, paraffin-embed (FFPE) block or slides must be submitted in order for Caris Target Now molecular profiling
to be performed.
Requisition Form Instructions:
Medical Oncology
1) Complete the Treating Physician, Patient and Pathology information sections, as well as list any additional physicians that need to
be copied when the final report is released.
2) Select the appropriate Billing process. Patient is billed directly, a separate form is provided with payment information.
An agreed payment method is required prior to testing. The institution is billed, prior or after tumour profiling is performed,
based on agreement.
3) Document the Specimen Information, including Primary Tumour Site, Specimen Site, Specimen block ID#, Date of Collection and
Pathology Diagnosis.
4) Choose either the Caris Target Now Comprehensive Profile, one Caris Target Now Select profile or individual assay box.
A molecular profiling service must be selected in order for testing to be performed.
5) Collect the pathology report and if possible clinical history/most recent notes and fax to +41 21 533 53 01 / +800 12 12 32 32
or email to [email protected].
6) Contact your pathology department to request specimen be sent to Caris Life Sciences for molecular profiling.

7) Place into the shipper kit: the specimen, pathology report, copy of the requisition form, and if possible clinical

8) Call FedEx to arrange pick-up.

Fixed Tissue
One (1) tumour-containing formalin fixed paraffin embedded block (FFPE) from most recent surgery
or biopsy. Tumour cells will be excised by microdissection from successive five micron sections of the
block until a total area of at least 15 mm2 is obtained for Caris Target Now Select profiles, and 30 mm2
for Caris Target Now Comprehensive.
