The College of Health Sciences (CHS) was established
in March 2005 under Executive Order 358 known as the Institutionalization of a Ladderized Interface between Technical Vocational and Training (TVET) and Higher Education (HE). IFSU was
mandated to offer the Ladderized BSN Program. The purpose of
Ladderization is to open pathways of opportunities for career and
educational progress of students
and workers. Specifically, it intends
to create a seamless and borderless
education and training system that
allows mobility in terms of flexible
entry and exit into the educational
system. Ladderization creates job
platforms at every exit and provides
the student an opportunity to earn income and have a decent life. In
2010, the Bachelor of Science in Nursing was revised from a ladderized program to straight nursing. This is in response to CHED
Memo 14. From 2007 to 2012 the College has produced esteemed
registered Nurses and Midwives. The Master of Arts in nursing was
also revised in 2011 to suit nursing challenges today.

Diploma in Midwifery Objectives
1. To equip students to become globally competitive in the
practice of the midwifery profession.
2. To develop the knowledge, skills and attitude of students
in the care of mother, child, adolescent and adult prior to,
during, and following pregnancy.
3. To enable the midwifery graduates to render high quality
primary health care services both in the community and
hospital settings.
The courses offered as approved by the Commission on
Higher Education (CHED) and the Board of Regents (BOR)
are:

2. BACHELOR OF SCIENCE IN NURSING (BSN) CMO 14 S. 2009
3. MASTER OF ARTS IN NURSING
-Major in Nursing Administration (with thesis)
-Major in Maternal and Child Health Care
4. 6 MONTHS CAREGIVING CERTIFICATE
FIRST YEAR
1.
Admission Requirements
Be a graduate of a general or vocational secondary
school duly recognized by the Department of Education
(DepEd)
2. Be admitted only to the first year level during the first
semester.
4. Not be a: (This applies to the Straight BSN program).
a.
graduate of a college course
b. shifter from other courses
c.
transferee from other school
5. Submit the following documents:
a. The original form 138 (high school report card)
which has not been cancelled by another school;
b. A certificate of good moral character executed by
the principal of the school where the applicant
graduated.
c. Four (4) copies of 2X2 photos taken within three
(3) months prior to applicant and;
d. Photo copy of birth certificate from NSO.
6. Submit a medical certificate which includes among others
the following:
a. That student has been found negative of hepatitis
disease
b. That the student has undergone pregnancy test and
found negative for pregnancy.
c. Students found negative of hepatitis disease must
submit herself/ himself for hepatitis vaccination
during the first semester of first year.
TRANSFER STUDENT
A student from other schools who wish to enroll in
the second, third and fourth year level of the BSN Curriculum
maybe admitted upon satisfaction of the following.
1. To be admitted to Second year, must have a general average rating of 80 %.
2. To be admitted to third year, must have a general average
rating of 85 %.
3. Have taken and passed all the general education and professional subjects and the pre-requisite as required in the
previous year level.
4. Has to submit the following;
a.
Transfer Credential
b. Certification of Good Moral Character
c.
Certificate of grades taken from previous school
5. Submit a medical certificate which include among others
the following:
a.

OLD STUDENT
Students who wish to enroll in the following semester must have taken and PASSED the required subjects and
the pre-requisites. A female student must have undergone
pregnancy test and is found negative for pregnancy.
