Dear Dr. (516) 663-2521 : This is to inform you of your appointment as a house officer at Winthrop-University Hospital (WUH), providing you fulfill all New York State and WUH requirements for admission to the postgraduate medical education program prior to commencement of the program. You are appointed to: Training Program: Training: 2) Date: 1) SAMPLE HOUSE STAFF CONTRACT For a one year period commencing: Stipend Level: PGY- Stipend Amount/annum: WUH Agrees: a) To provide an educational program that meets the standards as established by the Accreditation Council on Graduate Medical Education (ACGME). b) To provide remuneration biweekly as stipulated above. c) To provide all benefits and services as detailed on the Winthrop GME website posted at www.Winthrop.org, including: financial support, vacation, conference and sick time, parental leave, professional liability insurance, hospital and health insurance, disability insurance, living quarters, meals, laundry, counseling, medical and psychological support services, access to information related to eligibility for specialty board exams. d) To provide policies on: grievance and due process; harassment; physician impairment; leave of absence; effects of leave on ability of resident to satisfy requirements to complete program; residency closure/reductions; house staff reappointments, nonrenewal or non-promotion; moonlighting; duty hours; restrictive covenants; disability. These policies are described on the Winthrop GME website, posted at www.winthrop.org/departments/education/gme/gme-policies.cfm The House Officer Agrees: a) To meet the educational requirements and duties of the program as required by the Program Director and/or the Chief of Service, or their designee, and to fulfill resident responsibilities as described in the House Staff Manual and on the Winthrop GME website at www.winthrop.org/departments/education/gme/ap.cfm b) To provide safe, effective and compassionate care under the supervision of resident faculty. c) To comply with established WUH policies and ACGME, State and Federal policies, as described on the Winthrop website at www.winthrop.org `. d) To participate, as requested by the Hospital, in defense of any case occurring during your training or arising after completion of your training but having occurred during your training. Graduating house staff, as well as all other house staff leaving a training program at Winthrop, will be required to vacate Winthrop housing no later than 12:00 noon on June 25th of their final training year at the hospital. House staff graduating “off cycle” will be required to vacate Winthrop housing within 7 days of completion of their training period. Exceptions to this provision are granted only for extenuating circumstances related to completion of the required training period. All house staff living in Winthrop-owned housing will be required to carry renter’s insurance for the term of their stay in Winthrop housing. Documentation of this insurance must be provided to the Winthrop Housing Office annually. The term of this agreement is for one year only as specifically established above. Appointment to subsequent years will be dependent upon satisfactory progress in education, and satisfactory performance of all duties. The provisions of the current House Staff Manual are incorporated in this letter of appointment.
