PLEASE READ THIS BOOKLET CAREFULLY. It contains important information about Western Kentucky
University’s Benefit Plan effective January 1, 2015. This year’s enrollment begins Monday, November 3,
and ends Friday, November 21. All employees must actively enroll for 2015. If you do not complete the
enrollment process by November 21, you will be assigned default coverage.
WHAT'S NEW
It’s Time to Enroll – It’s Time to Get Active!
This year’s Annual Enrollment for 2015 benefits runs from November 3 through November 21. Everyone needs
to complete the enrollment process this year. We are introducing many changes and new choices, so your active
participation is the only way to ensure your 2015 coverage best meets your needs. Here’s a preview of what’s new
and what’s changing for 2015:
- We are introducing new health plans and wellness incentive options. When you make the Top Life Pledge, you’ll be enrolled in the “Healthy”
medical plan of your choice. One feature of the “Healthy” plans is a unique type of savings account to help you cover eligible health care
expenses and meet your deductible. The current High, Standard and Economy plans will not be available in 2015. Whether you’ll see an
increase or decrease in your monthly premium depends on the plan you elect, whom you cover, if you complete the Top Life Pledge.
- Spouses/Partners enrolled in the health plan will be included in wellness services and will be eligible to earn incentives.
- Know Your Rx Coalition (KYRx) will replace Anthem as the prescription drug plan administrator.
- Colonial will offer new Cancer and Medical Bridge policies designed to offer an additional level of financial security if you have high out-of-pocket
medical costs, are critically ill or experience a catastrophic event.
- The Vision plan will no longer be bundled with the WKU Health Plan. This means ALL employees and dependents, including those not covered by
the WKU Health Plan, will have access to vision coverage in 2015. Avesis will replace Anthem as the new vision provider.
- Participants on the Premier Dental Plan will experience a modest decrease in premiums for 2015 and the provider network is expanding.
- WageWorks will replace TASC as our Flexible Spending Administrator.
MAKING CHANGES
THROUGHOUT THE YEAR
The choices you make during Open Enrollment remain in place from January 1, 2015 through
December 31, 2015. You cannot add or drop coverage until the next Open Enrollment period in
November 2015 (for the plan year beginning January 2016) unless you have a change in family
status or experience another “qualifying event,” with benefit changes related to the specific life
event. The following events would allow you to make changes to your current benefits during the
plan year. You must notify the Employee Benefits section within 31 days of the “qualifying event.”
• Birth or adoption
• Marriage
• Divorce
• Employment Status (dependent loses/gains other coverage)
• Dependent no longer meets eligibility criteria

WHAT YOU NEED TO DO
Quit tobacco. If you’ve ever considered quitting tobacco, 2015
may be the time to do it! WKU will be evaluating implementation
of tobacco and non-tobacco rates over the next year. Take
advantage of the smoking cessation programs next year and
kick the habit.
Be active. Don’t just go through the motions this enrollment period.
Actively review the information and make your 2015 benefit decisions.
Make your Top Life Pledge. When you (and your spouse/partner) make
this wellness Pledge, you are eligible to receive wellness credits in your
HSA or HRA, along with discounted premiums.
Do the math. Premiums are changing for 2015, so be sure

designed to reward you for taking an active role in your
health and well-being and to slow the growth of health care
costs for you and WKU.
TAKE ACTION
Take action by November 21! If you do not enroll in medical
coverage before the November 21 deadline, you will be
automatically enrolled in the PPO/1,500 Plan with the same family
coverage category as your current election, or the waiver if you
currently waive coverage. In addition, you will not be eligible for
the WKU wellness credit or premium discount.
You will see innovative new health plan choices that come
with a type of savings account—either a Health Savings
Account (HSA) or Health Reimbursement Arrangement
(HRA)—to help you pay for eligible medical expenses. See
the comparison chart below for key differences between the
HSA and HRA.

The section below outlines the new health plans being offered in 2015. These plans replace the current High, Standard, and Economy PPO Plans.

NEW
HEALTHY
SAVER/2,600 PLAN
This plan is paired with a Health Savings Account (HSA), with a WKU initial credit of $800 or $1,300. You may also make voluntary pre-tax contributions
to your HSA. The HSA and WKU’s credits are designed to help you meet the higher deductible and out-of-pocket maximum of this plan. Here’s how it works:
• Regardless of your participation in the Top Life Pledge, WKU will make an initial credit to your HSA of $800 for employee only coverage or
$1,300 for employee + child(ren), couple and family coverage levels.
• If you make the Top Life Pledge when you enroll, WKU will credit your HSA with an additional $500 or $1,000 wellness credit and you will
receive a discounted premium of $15.00 per month for employee only and employee + child(ren) coverage levels or $30.00 per month for
couple or family coverage levels.
• You can make additional, voluntary pre-tax HSA contributions, up to annual IRS limits; employees age 55 or older may make additional catchup contributions.
• You use your HSA balance to help pay for eligible health care services for you and your enrolled dependents to help meet your deductible.
• Once you meet the deductible, the plan pays benefits as shown in the chart on page 6.
• Your HSA balance earns tax-free interest, it rolls over from year to year, and if you leave WKU you can take your HSA with you.
• If you decide not to participate in the Top Life Pledge, you will be enrolled in the “Saver/2,600 Plan” and will not be eligible for the additional wellness credits or the discounted premiums.

HEALTHY
PPO / 1,000 PLAN
This option generally has the highest employee premium but it also usually pays the highest level of benefits when you receive care. Here’s how it works:
• If you make the Top Life Pledge when you enroll, WKU will set up a HRA on your behalf and credit the account with $500 for employee only
coverage or $1,000 for employee + child(ren), couple and family coverage levels. You will also receive a discounted premium of $15.00 per
month for employee only and employee + child(ren) coverage levels or $30.00 per month for couple or family coverage levels.
• You cannot make voluntary contributions to your HRA; however, you can make voluntary pre-tax contributions to a Health Care FSA.
• You can use your HRA balance to help pay for eligible health care services to help meet your deductible.
• Once you meet the deductible, the plan pays benefits as shown in the chart on page 6.
• Your HRA does not earn interest; however, any remaining balance at the end of the year rolls over to the next year.
• If you leave WKU, you forfeit any remaining HRA funds.
• If you decide not to participate in the Top Life Pledge, you will be enrolled in the “PPO/1,000 Plan” and will not be eligible for HRA wellness
credits or the discounted premiums.

HEALTHY
PPO / 1,500 PLAN
This option is similar to the PPO/1,000, but it costs less each month because it generally pays the
lowest level of benefits toward the cost of services. Like the PPO/1,000 Plan, this option comes with
a HRA when you make the Top Life Pledge (see the plan above for how a HRA works). If you decide
not to participate in the Top Life Pledge, you will be enrolled in the “PPO/1,500 Plan” and will not be
eligible for HRA wellness credits or the discounted premiums.

IS "HEALTHY” IN YOUR
PLAN NAME?
It will be when you and your spouse/partner take the Top Life Pledge during this year’s annual enrollment process. For example, health plan names
for employees who take the Pledge are Healthy Saver/2,600, Healthy PPO/1,000 and Healthy PPO/1,500 Plans. If you don’t take the Pledge,
“Healthy” is dropped from the plan name.
OPTING
OUT OF HEALTH INSURANCE
If you waive health plan coverage, WKU will set up a Health Care FSA on your behalf. These funds may be used for reimbursement of eligible out-of-
pocket expenses as defined by the Internal Revenue Service (e.g., co-pays, co-insurance, deductibles, and vision and dental services). The Health Care
FSA may be used for eligible expenses incurred by all family members who qualify as a federal tax dependent. Here’s how it works:
• WKU will put $125 per month in the FSA for all employees who waive medical coverage.
• If you make the Top Life Pledge and complete the program requirements, you will be eligible to earn an additional $500 cash
incentive during the 2015 calendar year.

THE REWARDS OF WELLNESS
WKU is introducing exciting changes to the wellness program for 2015—including inviting your enrolled spouse/partner to participate in the Top Life
Pledge. When you pledge to take an active role in your health and well-being, you become eligible to receive a wellness credit to either your HSA or HRA
as well as discounted premiums on your health plan.
IT
STARTS WITH THE TOP LIFE PLEDGE
When you enroll for 2015 benefits, you’ll be asked to sign a
“Top Life Pledge” or decline to participate in the program for
yourself and your spouse/partner. Making the pledge is a new
step toward earning the WKU wellness credits and discounted
premiums. To maintain a discount on your health plan
premiums, you and your spouse/partner must take additional
action by September 1, 2015 (see Tier 2 Activities).
If you waive medical coverage, yet want to participate in
the program, you will receive a $125 monthly deposit into a
Health Care FSA plus additional cash bonus opportunities as
you successfully complete your commitments.

If you have ever considered quitting tobacco, 2015 may be the time to do it! The
Commonwealth of Kentucky and many major employers have implemented tobacco
and non-tobacco rates in their health insurance plans, and WKU will be evaluating
this component over the next year. WKU offers tobacco cessation through lifestyle
management coaching or through Cooper-Clayton group meetings offered oncampus. These tobacco cessation programs are available at no cost to participants
who complete the program requirements.

DRUGS
WKU is pleased to be joining the Know Your Rx
Coalition, a governmental purchasing collaborative,
in 2015. Through the Coalition, we join other
universities (e.g., the University of Kentucky,
University of Louisville and Eastern Kentucky
University) who are also interested in reducing
costs while increasing the level of service. By using
the economies of scale of the Coalition, WKU will
be able to maintain our own prescription drug
plan design (see right) while benefiting from cost
savings and the Coalition’s personalized assistance
for our employees. The Coalition is aligned with
Express Scripts.
Some preventive medications are covered in full

There will be modest enhancements to the Premier Dental Plan, now called the PPO
Plus Premier Plan, offered by Delta Dental. Members on the PPO Plus Premier Plan will

NEW
COLONIAL
LIFE
You have the option of electing an additional level of coverage through Colonial Life’s Medical Bridge and/or Cancer Policies. This coverage works with
WKU’s health plan by providing additional benefits for certain medical services such as extended hospital confinements or a cancer diagnosis, which can
be costly. If you wish to elect these benefits, you pay the full cost of this coverage.
• Colonial Life Group Medical Bridge Policy: This policy pays additional benefits for certain more costly health care services such as hospital
confinement and outpatient surgery. This type of coverage helps “bridge” coverage of your medical plan by offsetting some of your out-ofpocket costs, including expenses before you meet the deductible and the coinsurance.
• Colonial Life Group Cancer Policy: Cancer treatment can be expensive and lengthy. The benefits provided under this Policy can be used to help
you meet the health plan deductibles, pay the coinsurance or cover non-medical costs (e.g., lost income if you are not able to work during
treatment or certain travel expenses related to getting treatment.)
During Annual Enrollment, these policies are being offered on a guaranteed issue—meaning proof of good health is not required. Representatives
from Colonial Life will be at the Benefits Fair (November 3-6), if you are interested in learning more. In the meantime, visit www.wku.edu/benefits/bf for

NEW
LINCOLN FINANCIAL
LIFE
& DISABILITY
NEW for 2015: Lincoln Financial Group is offering guarantee issue on optional employee life and disability insurance, spouse life and short-term disability
as long as you enroll during the Annual Enrollment process. Guarantee issue means you do not need to provide proof of good health to enroll in this
coverage. Also starting in 2015, the optional employee life insurance premium will be changed from a pre-tax to a post-tax payroll deduction. This
change will offset current tax implications related to imputed income on benefit amounts in excess of $50,000.
Life insurance and short-term disability rates will not change in 2015, unless you move to a new age band. Age-banded rate charts are available online at
www.wku.edu/benefits/bf.
