Introduction to Concepts and Application: Diagnosis and
Treatment of Movement Impairment Syndromes of the
Elbow and Hand
Presenter: Cheryl Caldwell, PT, DPT, CHT
This course will present a review of selected concepts and
principles of the movement system impairment (MSI) theory
and its relationship to musculoskeletal pain syndromes.
The course includes both lecture and lab experience. It is
designed to improve the clinician’s skills in the performance
of the examination procedures and the recognition of signs
and symptoms of the elbow and hand movement system
impairment syndromes. The course will include: review of
muscle function for the elbow, wrist and hand; discussion of
the most common elbow and hand movement impairment
syndromes; and practice of selected tests in the examination of
the elbow, wrist, and hand.
Objectives:
Upon completion of this course the participant should be
able to:
1. Describe the key concepts and principles of examining
alignment and movement of the elbow, wrist, and hand.
2. Relate key concepts and principles to selected pain
syndromes of the hand and elbow: lateral epicondylalgia
and OA of CMC of thumb.
3. Describe the impairments induced by repeated movements
and sustained postures.
4. Describe the importance of active performance of precise
joint motion.
5. Describe individualizing the exercise program to include
correction of alignment and movement associated with
functional and fitness activities.
6. Discuss how impairments in muscle length, strength,
stiffness, and pattern of muscle activation contribute to the
relative flexibility of a joint.
7. Discuss muscle functions of the elbow, wrist, and hand.
8. Identify the movement system impairment syndromes and
contributing factors for selected pain syndromes of the
elbow and hand.
9. Perform selected tests in the examination to confirm/
disconfirm the diagnosis and to identify contributing factors.
10. Challenge the clinician to think in a new way about routine
tests used in the examination of movement of the elbow
and hand.
Fast and Frugal Examination and Treatment of the
Cervical Spine
Presenter: Chad Cook, PT, PhD, MBA, FAAOMPT
This is a demonstration, lecture-based course designed
to outline the most evidence-supported tests as well as
measures and treatments for the most common mechanical
pathologies of the cervical spine. The course will discuss a
standardized mechanical examination and introduce treatment
by classification. It involves Monday-morning knowledge and is
designed to be used immediately by all clinicians, regardless of
setting or level of experience.
Objectives:
By the completion of the course, participants will:
1. Compare and contrast the most common mechanical
pathologies of the cervical spine.
2. Categorize common conditions into treatment-based
classifications.
3. Analyze the evidence to support the majority of
examination and treatment approaches.
4. Evaluate the prognoses of selected conditions based on
baseline findings.
Best Tests for a Mechanical Diagnosis
Presenter: Chad Cook, PT, PhD, MBA, FAAOMPT
In clinical practice, physical therapists use tests and measures
for a probabilistic approach to clinical decision making. This
decision-making approach requires that the tests and measures
provide tangible value and can alter the post-test probability
of diagnosis or prognosis. Recent evidence also dictates that
selected tests should be used either early in the examination
process to rule out contenders or late in the examination
process to confirm hypotheses. Use of diagnostic accuracy
values such as sensitivity, specificity, and positive likelihood
ratios has improved our ability to discriminate tests’ strengths.
The focus of this course is to provide those tests and measures
as well as the appropriate order within an examination that
improve probability of a correct diagnosis or outcome.

Sharon Dunn, PT, PhD, OCS
Sharon Dunn is Associate Professor and Chair of the PT program
at LSUHSC in Shreveport, Louisiana, where she teaches
musculoskeletal content and professional issues. She has a PhD in
cellular biology & anatomy and carries out basic and clinical research
which includes topics such as osteoarthritis and coping mechanisms
for ACL deficiency. She is board certified in Orthopaedic Physical
Therapy and directs a post-professional PT residency in Orthopaedics
at LSU. In addition, she is passionate about our profession, currently
serving as the Vice President of the APTA. In that role she serves
on the Executive Committee, chairs the Public Policy and Advocacy
Committee, and is APTA’s liaison to the Federation of State Boards of
Physical Therapy.
Cheryl Caldwell, PT, DPT, CHT
Dr. Caldwell is an associate professor of physical therapy and
orthopaedic surgery at Washington University School of Medicine in
St. Louis, Missouri. Cheryl maintains a clinical practice at the School
of Medicine specializing in posture and movement as they relate to the
treatment of musculoskeletal conditions, with a particular emphasis
on the upper quarter, including hand, elbow, shoulder and neck
pain. She is a certified hand therapist. Cheryl received her Master of
Health Science and her Doctor of Physical Therapy from Washington
University and her Bachelor of Science in Physical Therapy from the
University of Colorado in the United States of America.
She has published a case report using a Movement System
Impairment Diagnosis to guide treatment for a patient with shoulder
pain and co-authored two chapters on the hand and elbow. She is
currently working as a co-author on revising the shoulder chapter for
the second edition of the text Diagnosis and Treatment of Movement
Impairment Syndromes.
Chad Cook, PT, PhD, MBA, FAAOMPT
Dr. Cook is a professor and vice chief of research at Duke University in
Durham, North Carolina. Dr. Cook received his Bachelor of Science in
physical therapy at Maryville University in 1990, a Master of Business
Administration in 1999, and a Doctorate of Philosophy at Texas Tech
University in 2003. He was certified as an orthopedic manual therapist
in 2001 and received fellowship status at the American Academy of
Orthopedic Manual Therapy in 2006. Dr. Cook has published over 165
peer reviewed papers, three textbooks (two in their 2nd edition and
one in its first edition), and several chapters in other textbooks. He has
presented internationally at over 145 conferences and events. He is
an associate editor with Manual Therapy and is on the editorial staff of
several other international journals. Dr. Cook won numerous awards
locally at Duke for teaching as well as the OPTP/JMMT literature
review award in 2003 and 2005; the 2009 top educator award for the
American Physical Therapy Association; the 2011, 2012, and 2013
excellence in research award at AAOMPT; the 2008 Helen Bradley
career achievement award and the 2005 J Warren Perry Distinguished
Authorship Award.
PTA Speaker - James Laughlin, PTA, BS
James graduated from Mercy College of Health Sciences in the
Physical Therapist Assistant program. He also earned his Bachelors
degree in Exercise & Sport Science from Iowa State University. James
is Clinic Director at Physiotherapy Associates in Johnston, Iowa;
center coordinator for clinical education; and credentialed clinical
instructor. James also is a board member of the Mercy College Alumni
Association. He specializes in sports medicine as well as general
orthopedics.

Ames, IA 50014. It is a short 5-minute drive from the Scheman Building.
