PERFECT HEALTH SURVEY
LONG TERM CARE
The Quality Indicatory Survey (QIS) is the long-term care
survey process used to determine compliance with
Medicaid and Medicare certification standards. It is a
resident-centered, outcome-oriented quality review which
entails structured resident, family, and staff interviews,
resident observations, record reviews, and analysis of
health assessment data.
We recently had our survey in which the Ohio
Department of Health found ESC to be in compliance
with all health and licensure regulations. Congratulations
to ESC on achieving a deficiency free survey and for
providing excellent care to our residents.
NEW REHAB WING PROGRESS!
On Tuesday, November 11, 2014, at
8:30 a.m., we will hold a special Veterans
Day breakfast, in the Friendship Center,
as we salute our brave residents (listed
below) who have served in the military. In
addition, we will continue honoring our
veterans that afternoon at 2:00 p.m. in the
Friendship Center with a special performance
by the Sylvania Senior Center Choir.

Friday, November 28, 2014, the Activities
Department will spend the entire day
decorating the inside of the facility for
Christmas in lieu of group resident activities.
Residents along with family members are
encouraged to take this opportunity to
decorate the inside of their apartments or
rooms, as well as their doors to celebrate the
upcoming holiday.
The Level I residents even have a
door-decorating contest which helps bring a
joyful look throughout the hallways.
If you would like to volunteer and help the Activity
Department on this day, please contact one of the
Activity Coordinators.
