Licensed Vocational Nurse
LVN I – 1 Temporary Fulltime Position – 100% Time – San Fernando Health Center
– Work schedule: TBD, work hours may include evenings and Saturdays
 LVN II - 1 Regular Fulltime Position – 100% Time – Maclay Health Center – Work
schedule: TBD, work hours may include evenings and Saturdays
 LVN I - 1 Regular Fulltime Position – 20% Time – San Fernando Health Center –
Work schedule: Mondays & Tuesdays, 5:00pm – 9:00pm

Definition:
The Licensed Vocational Nurse (LVN) I/II/III provides a variety of
administrative, clinical nursing, health education, and outreach services
within the community health center. Basic skills for physical assessment
include observation, auscultation, palpation and percussion. Basic mental
status assessment includes observation of general behavior, appearance and
attitude, collection of data relative to sensorium or orientation to time,
person and place, and assessment of recall ability. When the LVN notes a
problem, complication or abnormality, he/she must notify the Physician or
Registered Nursing Supervisor under whose direction the licensee functions.
The LVN cannot independently evaluate or synthesize data.

Qualifications:

2. Satisfactory completion of a state approved program in vocational nursing, plus
current license to practice in the State of California.
3. Must be able to communicate tactfully and empathetically, with cultural
sensitivity; with staff and patients. Fluently bilingual English + Spanish required,
oral and written communication skills demonstrated in both languages.
4. Valid CA Driver’s License, current vehicle liability insurance, and an available
vehicle required.
5. Must possess excellent interpersonal skills.
6. Must maintain a current Cardio-Pulmonary Resuscitation certification from the
American Heart Association.
7. Must be willing and able to work a varied and flexible schedule to accommodate
the needs of NEVHC and its patients.
8. Must maintain or exceed competency levels as established by corporate standards
for clinical practice.
9. Must demonstrate the knowledge and skills necessary to provide appropriate care.
10. The MIS Patient Information System access level is 2.
Lead Duties: May lead a team of Medical Assistants, Team Clerks, Community Health
Workers, and other unlicensed assistive personnel as appropriate.
Responsibilities:
1. Performs, under the direction of the RN Supervisor or Program Manager,
delivering technical nursing services in caring for acute or chronically ill
patients, including:
(a)
Assists provider with examinations or special procedures, such as
lumbar punctures, sigmoidoscopies, etc.
(b)
Administers medications orally, topically, and by injection.
(c)
Provide nursing services, including routine screening services, (e.g.:
EKG, hearing and vision assessments, and immunizations).
2. Communicates information to the RN Supervisor or Program Manager through
observation of the patient physical condition and behavior, signs and symptoms
of illness and reactions to treatment and through communication with the patient
and other team members.
3. Executes interventions in accordance with patient’s care or treatment plan.
4. Contributes to evaluation of individualized interventions related to care or
treatment plan.
5. Serves as the nursing team leader and coordinates the activities of Medical
Assistants and Team Clerk in the absence of the Registered Nursing Supervisor
or Program Manager.
6. Initiates emergency procedures and coordinates the activities of Medical
Assistants and Team Clerks until a provider or registered nurse arrives.
Participates in planning and executing emergency code drills. Performs CPR as
needed.

7. Assembles appropriate and required chart data and provides basic physical
assessment of client health problems.
8. Maintains required logs and statistics (e.g.: CHDP, hospital, and referral logs)
9. Participates in the in service training of unlicensed assistive personnel and the
orientation of new team members.
10. Applies communication skills for the purpose of patient care and education.
11. Contributes to the development and implementation of a teaching plan related to
self-care for patients and provides education and reinforcement to the patient for
the self-care.
12. Participates in patient follow-up activities for missed appointments, abnormal
laboratory results, and maintains patient tickler file.
13. In the absence of the lab assistant/phlebotomist, draws blood specimens and
performs routine tests, including hematocrit, dipstick urinalysis, and the
preparation of other specimens.
14. Participates in routine audits of patient medical records to determine
appropriateness of nursing care, documentation, and follow-up plans.
15. Perform other duties as may be required or assigned by the RN Supervisor,
Program Manager, Clinic Administrator, or Director of Nursing Services.
16. Performs clinical specialty area related duties as required (e.g.: Pediatrics, Adult
Care Family Planning, Prenatal, Adolescent Medicine, and Care of HIV plus,
etc.)
17. Participates in and complies with all NEVHC mandatory trainings including the
following:
a. Infection Control Program
b. Environment of Care Management Plans and Emergency Management
Program; and participates in emergency response activities as directed
c. Corporate Compliance Program
d. Code of Conduct
e. Risk Management & Compliance
18.
Participates in hazardous waste and infection control assignments as
required which may include being designated as an emergency responder to
a hazardous substance release or spill; performing infection control data
collection, evaluation, reporting and follow-up in accordance with clinical
health services policy and procedures.
