This form must be signed by a parent or guardian of each contestant and received by the Virgin
Islands Caribbean International Classic Coordinator before the contestant's entry will be processed. If
the contestant is over the age of 18 years, the contestant must sign the Participant's Liability
Release Form.
I (We) represent that we are the parents or legal guardian of the (competitor), twirler, minor, under
the age of 18 years, and in order to induce you to accept the competition entry, have accepted this
document on behalf of said minor and ourselves individually and jointly, and agree to be bound by all
the terms and conditions of this liability release, in the same manner as if we were the (competitor),
twirler.
Each of us hereby releases the Virgin Islands Caribbean International Classic, its officer, agents
and/or employees from any and all liabilities. With the acceptance of this form, the undersigned
hereby agrees to indemnify and hold harmless the Virgin Island Caribbean International Classic,
officers, agents and/or employees from any liability in connection with the Virgin Island Caribbean
International Classic.
