While Medicare and healthcare insurers look for ways to shorten hospitalization and maximize recovery of patients in their
homes, rehab in the home healthcare setting will continue to grow as our population ages. In addition, rehabilitation and major
joint & limb procedures have been listed as the top two most common reasons behind acute discharges to homecare. Financial
incentives that call for reduced re-hospitalization rates, quality outcomes and effective documentation will also impact home
health rehab reimbursement. Home health therapists across the nation need up-to-date knowledge and resources to provide
quantified effective interventions to their patients while optimizing patient satisfaction, improving quality outcomes and ensuring
reimbursement.
This important workshop is designed to provide rehab therapists and managers with the knowledge and tools needed to
establish an effective rehab program in the home health setting based on quality measures and documentation requirements
from Medicare. Topics include discussion of current Medicare regulatory changes relevant to home health rehab and managing
quality domains related to acute re-hospitalizations and falls prevention. Participants will learn OASIS guidelines for functional
scoring and documentation recommendations to maximize reimbursement. Gaining the knowledge and tools for setting up plans
of care will allow participants to be more effective as rehab professionals, and deliver quality care in the home health setting.
About Your Program Instructor
Karen Yong, MA, PT, is a licensed physical therapist with thirty years of clinical and management experience in home health,
acute care, skilled nursing facility (SNF) and outpatient settings. Ms. Yong is the owner/director of HOME Rehab Services where
she provides home-based physical therapy, occupational safety, management/consultation and educational services. Her
passion for homecare, rehab management and injury prevention is exemplified in her twenty years with Kaiser Home Health
as a Rehab Manager and with Kindred (formerly Vencor) as an Area/Rehab Manager. Ms. Yong has implemented and managed
injury prevention programs, assisted in the formation and certification of a home health agency (HHA) as well as developed
and managed the rehab services for several HHAs. She has hired and trained more than two-hundred therapists and is known
for her clinical excellence, leadership, management, and program development expertise. In addition to leading seminars for
healthcare professionals, certified home health aides and patient groups, Ms. Yong has published articles related to stroke
rehab in SNFs as well as home safety for patients going home from post-acute settings. Ms. Yong has been an ancillary advisor
for the California Association for Health Services at Home and has served on several home health professional advisory boards
including her current role with the city and county of San Francisco’s HHA. Ms. Yong received her BA in Physical Education from
the University of California, Berkeley and her MA in Physical Therapy from Stanford University. She is a member of the American
Physical Therapy Association and the Home Health Section.
