In addition, rehabilitation and major joint & limb procedures have been listed as the top two most common reasons behind acute discharges to homecare. Financial incentives that call for reduced re-hospitalization rates, quality outcomes and effective documentation will also impact home health rehab reimbursement. Home health therapists across the nation need up-to-date knowledge and resources to provide quantified effective interventions to their patients while optimizing patient satisfaction, improving quality outcomes and ensuring reimbursement. This important workshop is designed to provide rehab therapists and managers with the knowledge and tools needed to establish an effective rehab program in the home health setting based on quality measures and documentation requirements from Medicare. Topics include discussion of current Medicare regulatory changes relevant to home health rehab and managing quality domains related to acute re-hospitalizations and falls prevention. Participants will learn OASIS guidelines for functional scoring and documentation recommendations to maximize reimbursement. Gaining the knowledge and tools for setting up plans of care will allow participants to be more effective as rehab professionals, and deliver quality care in the home health setting.
