How to sign up
1. You will receive an e-mail with a link to the NextGen Patient Portal from your healthcare provider.
2. To enter the Portal, clink on the link, review the Terms and Conditions, and then click the “I
accept” button
3. Enter the security token you received from your healthcare provider, as well as your e-mail
address, and click the “Submit” button
4. If this is the first time using the Patient Portal, click the “Enroll Now” button to create your user
name and password
Enter your enrollment token and your email address. Verify that your email address is correct;
otherwise you will not be able to enroll in NextMD. Contact your practice if your email address is
incorrect.
Choose a user name and password. Your user name and password are case sensitive. Your user
name must be at least 6 characters long and cannot be more than 20 characters long. Your
password must be at least 6 characters long, cannot be more than 20 characters long, and must
contain at least one number.
Choose a Login Security Question from the list and provide your answer for this question. Since
your NextMD account contains confidential medical information that must be protected you will
have to provide the answer to the Login Security Question whenever you log into NextMD.
Set up NextMD's secure password reset feature. NextMD will ask you to select a question and
provide an answer to it. If you forget your password, you will have to answer this question again
to reset it.
5. If you already created an account, enter your user name and password and click the “Log in”
button
6. Answer the security question and click the “Submit” button
How to Send a Secure Message to your Health Care Team
1. In the left hand navigation bar, under “Tasks”, click the “Compose Message” option
2. Select a category from the drop-down i.e., Medical Questions
3. Select your healthcare provider from the “To” drop-down
4. Enter a subject in the “Subject” field
5. Fill in your message in the “Message” box and click the “Submit” button
How to Request an Appointment
1. In the left hand navigation bar, under “Tasks”, click the “Request Appointment” option
2. Select a the name of the doctor you want to see, a category and a location
3. Enter a reason for visit and select preferred dates and times of the appointment
4. Click the “Submit” button when done
How to request a medication refill
1. In the left hand navigation bar, under “Tasks”, click the “Renew Medication” option
 You will see a list of active medications prescribed by your healthcare provider
2. Select the medication you would like to refill (you may choose more than one)
3. Select a pharmacy
4. Complete all the required fields displayed with an asterisk
5. Click the “Submit” button when done
