2014 CMS-2744 Form
• Facilities may begin the CMS-2744 completion process if all
2014 patient events and forms are in CROWNWeb.
• ALL facilities must complete the CMS-2744 form in
CROWNWeb.
• Facilities must resolve CMS-2744 errors and work to finalize
their forms.
• Facilities must use built-in CMS-2744 reports to resolve errors
that may occur.
• Facilities must work with their ESRD Networks to finalize their
CMS-2744 forms.
• CMS-2744 training events are open to ALL users.

• Introduced to facilities in 2008, as part of the April 15, 2008
Updated ESRD Conditions for Coverage for End-Stage Renal
Disease Facilities.
• Section 494.180(h) called for furnishing data and information
for ESRD program administration via an electronic format.
• CROWNWeb was designed to support:
 ESRD administrative forms;
 Patient survival information; and
 Existing ESRD clinical performance measures, and any future
clinical performance standards developed in accordance with a
voluntary consensus standards process identified by the Secretary.

Phased-In Release
• CROWNWeb was initially released February 1, 2009 via a
“phase-in approach.”
• CMS continued to test CROWNWeb and implemented
updates prior to the full National Release.
• CMS solicited feedback from users to enhance CROWNWeb
based on their needs.
• CROWNWeb was released nationally for use by all Medicarecertified dialysis facilities on June 14, 2012.

System Usage
• Since its June 2012 National Release, CROWNWeb has been
used by thousands of authorized individuals.
• QIMS account statistics:
 Over 29,000 active QIMS Accounts
 Approximately 18,150 unique QIMS users
• CROWNWeb currently holds hundreds of thousands of ESRD
patient and clinical data.
• Overall average for clinical compliance is 97% or greater!

Supporting CMS Initiatives
• CROWNWeb supports a multitude of CMS initiatives.
Patient events and clinical data backs:
 CMS’ goal to optimize health outcomes as part of the CMS Quality
Strategy.
 Efforts to promote high-quality patient care through the ESRD
Quality Incentive Program (QIP).
 Endeavors highlighted as part of the CMS Triple Aim.

• CMS implemented three versions of CROWNWeb in 2014.

• These releases included many features requested by facilitylevel users.
• Screens in CROWNWeb were streamlined and fields were
removed to further support CMS’ data collection and reporting
initiatives.

Challenge
and Resolution
Challenge and Resolution
• Monthly clinical carry-over was one of the most requested
CROWNWeb enhancements.
• Clinical carry over capabilities had been on CROWNWeb’s
Vascular Access screen since December 2012.
• CMS attempted to expand this feature to fields on the
Hemodialysis and Peritoneal Dialysis Clinical screens in May 2014.
Challenge
• An error occurred that did not allow all the carry over
fields to display from month-to-month.
Resolution
• CMS worked with its CROWNWeb support teams and
identified the root cause of the error.
• CMS implemented a fix as part of CROWNWeb v4.6 in
December 2014.

Planned CROWNWeb Release
• CMS plans to release CROWNWeb 4.7
in May 2015.
• CROWNWeb 4.7 will include behind the
scenes updates, technical upgrades, and 3 small
enhancements
• Updates are designed to improve user
experience.
• Scope decisions for a 4.8 release are still pending
approval and scheduling.

Data Management Guidelines
• Thousands of authorized individuals use CROWNWeb to
submit and access data.
• Certain authorized individuals are responsible for
CROWNWeb data management and need to understand
and begin to utilize standardized processes.
• CMS is developing the CROWNWeb Data Management
Guidelines to assist with the following data governance:
 Data Monitoring
 Data Measurement

ICD-10 Implementation
• The United States is preparing
to adopt the Tenth Revision of
the ICD in October 2015.
• CROWNWeb has been updated to reflect ICD-10 Codes
for the “Primary Cause of Renal Failure” field on the
CMS-2728 form.
• The CMS-2728 form was updated in July 2014 in
preparation for the ICD-10 transition.
• ICD-10 codes will automatically activate in CROWNWeb
on October 1, 2015.
