General Rules:  Vendors are responsible for any KY Sales Tax and/or locally required vending licenses.  Vendors may only sell items approved on this application.  Failure to abide by rules will result in removal from event with no refund of fees.  No alcoholic beverages to be sold by vendor.  Beverages must be in paper-plastic-aluminum cups, bottles or cans. NO GLASS  Local health inspectors will be on site.  Vendor will provide a copy of a Certificate of Insurance for liability purposes showing at least $1 million in coverage. Said certificate shall list Bourbon City BBQ Festival, Inc. as additional insured.  You must be set up by 5 p.m. on Friday, 6/5 and cannot tear down before 10 p.m. on Saturday, 6/6. All applications must be in by May 15, 2015. You must provide a photo of your booth along with menu prices and a picture of your electrical plug to be used. You must provide your own electrical cord (100ft) and water hoses (100ft). Please advise Amps ______and Voltage ______ needed. I hereby agree to release any rights and claims for damages and waive any claims for liability on behalf of myself, my heirs, executors and administrators, which I may have against the event, its organizers, sponsors and their affiliated companies, officers, agents and employees. Bourbon City BBQ Festival, Inc. will be free from any liability for any claims for damages or injuries arising from operation of the food booth or from products sold by vendor. I also grant permission for the use of any images, photographs and other media record of this event for any legitimate purpose concerning this event.
