The Patient Care Coordinator assists the patients' entry into and through the health care system facilitating the delivery of optimal patient care by proactively arranging scheduling and transportation for appointments and working in the Care Team to meet the patients’ psychosocial needs. The Patient Care Coordinator is a conduit for the transmission of information between licensed providers, patients, families and social services. The Patient Care Coordinator will provide advocacy, information and referral services to patients and families. Essential Functions and Responsibilities Respects patients by recognizing their rights; maintaining confidentiality Contributes to the team effort by accomplishing delegated tasks on time Provides information and education to individuals and groups in the community, as required and requested. Encourage patients and families to be involved in care, if appropriate Encourage patients and families to take responsibility for care, when appropriate Work with all family, medical and other resources, as required Advocate on behalf of clients for additional service and resources, as required Establish and maintain current, accurate, confidential files for each beneficiary Educate patients, clients, families and support systems as directed Qualifications: Associates’ or Bachelor’s Degree in a health care related field 2 years’ customer service experience 2 years’ patient care experience, Medicare patients preferred Requirements: Is highly organized, effective at managing her/his time, and is skilled in the use of computers and multiple software systems. Remains objective in relationships with patients, families and physicians and promotes a team approach by encouraging communication among all members of the care team. Coordinates care among multiple treating physicians including specialists, primary care, hospitalists and behavioral health care, as necessary. Excellent professional written and verbal skills Excellent customer services skills Ability to function in a team with patients, families, physicians, MD office & UR department staffs. Able to ask for assistance when needed Adaptability for change Compensation: This position is classified as non-exempt, full-time regular. We have a generous benefits package.
